Commissioned in 1932, the original Route 159 was Cherry Hill Road in Middlefield, a north-south road from Route 157 to present-day Route 147, which was back then part of Route 157.
In the 1920s, much of it (except for a western jog along the original Route 75) was called state highway 110.
