Acarbose Treatment Decreases Risk of Cardiovascular Disease - February 1, 2004 - American Family Physician
Acarbose therapy increased the probability that patients would remain free of cardiovascular events (hazard ratio of 0.51), with 32 in the placebo group and 15 in the acarbose group having at least one cardiovascular event.
Acarbose treatment lowered the risk of developing hypertension, with 78 patients (11 percent) developing hypertension in the acarbose group, compared with 115 (17 percent) in the placebo group, a relative risk reduction of 34 percent and an absolute risk reduction of 5.3 percent.
Acarbose was associated with a significant reduction in cardiovascular events in a group of participants with impaired glucose tolerance.

Thirty percent of the acarbose group and 18 percent of the placebo group discontinued the study.
The authors call for acarbose to be used either alone or in combination with lifestyle modifications to delay the onset of diabetes in patients with impaired glucose tolerance.

Acarbose is a man-made oligosaccharide designed to slow down the actions of alpha-amylase and alpha-glucosidase enzymes thereby slowing the appearance of sugar in the blood after a meal.
Acarbose can be used alone to treat type II diabetes or can be combined with sulfonylureas such as glyburide (Diabeta) or metformin (Glucophage) or with insulin.
Acarbose therapy is not advised in the presence of certain medical conditions such as inflammatory bowel disease or intestinal obstruction and chronic intestinal diseases that interfere with digestion or absorption such as Crohn's disease.

If mild to moderate hypoglycemia occurs while taking acarbose in combination with another anti-diabetic agent, the hypoglycemia should be treated with oral glucose (dextrose) instead of sucrose (table sugar) because acarbose blocks the digestion of sucrose to glucose, and hypoglycemia will not be corrected rapidly with sucrose.
Acarbose (also known as Glucobay ®) is a drug that is used in the treatment of diabetes because it helps control blood sugar levels by slowing down the digestion of carbohydrates (simple and complex starches/ sugars) from the diet.

Structurally, acarbose represents a pseudotetrasaccharide, consisting of an unsaturated aminocyclitol, valienamine, a deoxyhexose and maltose.
The pseudodisaccharide core structure, known as acarviosine, which is composed of valienamine and the deoxyhexose, is postulated to be essential for its biological activity.
