Topic: Acetylcysteine

  s010910a - Acetaminophen Poisoning - Late Administration of N-acetylcysteine
Acetylcysteine treated patients had a lower incidence of cerebral oedema (40% (10/25) v 68% (17/25); p = 0.047, 95% confidence interval for difference in incidence 2% to 54%), and fewer developed hypotension requiring inotropic support (48% (12/25) v 80% (20/25); p = 0.018, 95% confidence interval 7% to 57%).
Abstract: The influence of acetylcysteine, administered at presentation to hospital, on the subsequent clinical course of 100 patients who developed paracetamol-induced fulminant hepatic failure was analysed retrospectively.
Acetylcysteine was effective in preventing liver damage, hepatic failure, renal damage, and death when given eight to ten hours after poisoning.

Acetylcysteine is the N-acetyl derivative of the amino acid L-cysteine, and is a precursor in the formation of the antioxidant glutathione in the body.
Oral acetylcysteine for this indication is uncommon as it is poorly tolerated owing to the high doses required (due to poor oral bioavailability), unpleasant taste/odour and adverse drug reactions (particularly nausea and vomiting).
Oral acetylcysteine is used for the prevention of radiocontrast-induced nephropathy (a form of acute renal failure).

 Drug Information
Acetylcysteine makes mucus or sputum in the lungs easier to cough up; it can help improve breathing in patients with COPD (chronic bronchitis or emphysema) or other lung diseases.
Acetylcysteine is also used by doctors as an antidote for treating overdoses of acetaminophen (Tylenol(R)); it can help reduce the amount of liver damage caused by acetaminophen.
Acetylcysteine can interact with certain metals and rubber; do not mix the oral solution in containers made of these materials.

BioMed Central | Full text | Acetylcysteine for prevention of contrast-induced nephropathy after intravascular ...
Acetylcysteine was associated with a significantly reduced incidence of contrast-induced nephropathy in five studies, and no difference in the other nine (with a trend toward a higher incidence in six of the latter studies).
The potential benefit of acetylcysteine is believed to be mediated by its properties as a scavenger of free-radical species and by increasing the synthesis of nitric oxide, a potent vasodilator, in response to ischemic or other toxic injury in the kidney [12].
The dosing and schedule of administration of acetylcysteine was variable across studies; however, in the majority of studies, acetylcysteine was initiated 12–24 h prior to angiography.

Acetylcysteine : by Ray Sahelian, M.D., health benefits of N-Acetyl-l-cysteine
Acetylcysteine treatment was discontinued when the APAP concentration was <10 mg/L and serum aspartate aminotransferase (AST) was <40 IU/L. For the purposes of assessing clinical outcomes, patients were divided into 3 groups based on duration of treatment: the short-course group (/=73 hours).
Acetylcysteine was intravenously infused for 20 min prior to and throughout exercise.
In addition to its antioxidant properties, acetylcysteine is currently used in a variety of other ways: to counteract the effects of an overdose of acetaminophen (i.e., Tylenol®), to break up mucus in respiratory ailments, to lessen the symptoms of colds or the flu, and even to reduce the effects of hangovers.

Both solutions were a water in oil emulsion, one of which contained 10 per cent acetylcysteine and the other (with no additions) was a placebo.
Dr Redondo concludes that because acetylcysteine is an atoxic and hypoallergenic amino acid derivative with successful therapeutic uses and rare side effects, "it may be useful in the treatment of hyperproliferative skin disorders."
