Lymphoma Information Network - Adriamycin / Doxorubicin
Adriamycin (generic name doxorubicin) is a common chemotherapy agent in the treatment of Hodgkin's Lymphoma / Disease and Nonhodgkin's Lymphoma.
It is is a cytotoxic anthracycline antibiotic isolated from cultures of Streptomyces peucetius var.
As such, it is an antineoplastic - adriamycin interferes with the growth of cancer cells.

Adriamycin-induced DNA Adducts Inhibit the DNA Interactions of Transcription Factors and RNA Polymerase -- Cutts et al. ...
Although the reactivities of adriamycin with a range of cellular constituents are well known, the specific cellular mechanisms involved and the ultimate cause of tumor cell death are still to be elucidated.
At 10 µM adriamycin, the binding of Oct-1, N-Oct-3, and N-Oct-5 is inhibited by approximately 25, 40, and 30% respectively (Fig.
Although adriamycin has been a major inclusion in chemotherapy regimes for 20 years and appears to act as a topoisomerase II inhibitor, a major research effort is still required directed toward elucidating its basic modes of action in tumor tissue.

Guangzhi Qu, MD, a fellow in oncology at the Mayor Clinic, in Rochester, Minnesota, said that four of 16 patients with advanced prostate cancer had partial responses to the treatment-meaning that prostate specific antigen (PSA) levels dropped by at least 50 percent or that tumors decreased in size by 50 percent.
The 24 patients who received the dose-escalating regimen of adriamycin while receiving a dose-lowering schedule of suramin had various types of advanced cancer, but the only partial responses seen were in those patients with prostate cancer.
Adriamycin was dosed a 10 mg for the first patients in the study, and was increased to 60 mg for the last patients entered into the study.

The study included 429 women with documented metastatic breast cancer who had not undergone prior chemotherapy for metastatic disease and had not received a prior anthracycline-containing regimen as adjuvant and/or neo-adjuvant chemotherapy.
The regimen was repeated every three weeks for a maximum of eight cycles without the use of prophylactic colony-stimulating factors (agents used to strengthen the body's ability to fight infection) or antibiotics, except following an episode of neutropenia (a low white blood cell count).
Of particular note, the rate of cardiac toxicity for the two treatment combinations did not exceed that expected with the total cumulative doses of Adriamycin given.

Single Dose of Adriamycin Triggers Acute Left Ventricular Failure - CME Teaching Brief - MedPage Today
Cardiotoxicity is a well-recognized side effect of Adriamycin treatment, but typically heart failure develops over time and is dose dependent.
This case report suggests that Adriamycin may also trigger acute ventricular failure, and the authors suggest that ventricular function should be assessed in patients who develop pulmonary symptoms after even a single dose of Adriamycin.
AKRON, June 8-A single 50-mg dose of Adriamycin (doxorubicin) during hepatic artery chemoembolization for a metastatic carcinoid tumor resulted in acute reversible left ventricular failure in a 78-year-old woman, clinicians here reported.

Herceptin Without Adriamycin Safer for the Heart   (Site not responding. Last check: 2007-10-08)
Background and importance: Studies in the past showed that the combination of Herceptin and Adriamycin (chemical name: doxorubicin) could weaken the heart muscle, putting a woman at increased risk for congestive heart failure.
If a woman with breast cancer had a very good response to Adriamycin and Herceptin, but she develops some weakening of the heart, then she needs to stop the medications, at least temporarily.
The study suggests that if you've had heart trouble with Adriamycin with or without Herceptin, and your heart has gone back to normal, you might be able to start taking Herceptin (or take it again if you already have in the past) without damaging your heart more.
