Topic: Agnogenic myeloid metaplasia

Agnogenic Myeloid Metaplasia - New Treatments, January 2, 2007
Myelofibrosis in agnogenic myeloid metaplasia (AMM) is a secondary reaction to the clonal hemopathy.
AMM occurs mainly in middle-age and elderly patients (median age at diagnosis is 60), and men and women are affected with equal frequency.
AMM may be asymptomatic in as many as 25% of patients, and the diagnosis is made after investigation of abnormal blood findings or the discovery of splenomegaly.

The terms "myeloid metaplasia" and "extramedullary hematopoiesis" are used to describe a pathologic process of ectopic hematopoietic activity that may occur in any organ system but that primarily affects the liver and spleen.
Myeloid metaplasia is not always associated with myelofibrosis, and either of these processes may occur in the absence of a clonal hematological disorder.

Agnogenic myeloid metaplasia (AMM) is one of the Philadelphia chromosome negative myeloproliferative disorder and is diagnosed by hyperplasia of atypical megakaryocytes, hepatosplenomegaly, extramedullary hematopoiesis and bone marrow fibrosis.
Agnogenic myeloid metaplasia (AMM), polycythemia vera (PV), essential thrombocythemia (ET) and chronic myelogenous leukemia (CML) are characterized as myeloproliferative disorders (MPDs) arising due to exponential amplification of a haematopoietic stem cell.
AMM, one of the Ph negative MPD is diagnosed by hyperplasia of atypical megakaryocytes, hepatosplenomegaly, extramedullary haematopoiesis and bone marrow fibrosis.

Agnogenic myeloid metaplasia (AMM, called chronic idiopathic myelofibrosis (CIMF) in the WHO classification and primary myelofibrosis by the International Working Group on Myelofibrosis Research and Treatment), is one of the chronic myeloproliferative diseases.
AMM, PPM, and PTM are all referred to as myelofibrosis with myeloid metaplasia (MMM).
The primary disease process in AMM is a clonal hematopoietic stem cell disorder which results in chronic myeloproliferation and atypical megakaryocytic hyperplasia.

Agnogenic myeloid metaplasia is characterized by splenomegaly, immature peripheral blood granulocytes and erythrocytes, and teardrop-shaped red cells.
As distinguished from chronic myelogenous leukemia, agnogenic myeloid metaplasia usually presents with a white blood cell count less than 30, 000 per cubic millimeter, prominent teardrops on peripheral smear, normocellular or hypocellular marrow with moderate to marked fibrosis, and an absence of the Philadelphia (Ph') chromosome.
Untreated agnogenic myeloid metaplasia means that a patient is newly diagnosed and has had no prior treatment except supportive care.

The myeloproliferative diseases are four conditions- essential thrombocythemia, polycythemia vera, chronic myelocytic leukemia, and agnogenic myeloid metaplasia- characterized by overproduction of normal-looking blood cells.
Agnogenic myeloid metaplasia (idiopathic myelofibrosis, myelofibrosis with myeloid metaplasia) begins like other myeloproliferative diseases, with overproduction of blood cells.
Essential thrombocytopenia, polycythemia vera, and agnogenic myeloid metaplasia are usually diagnosed late in life, at an average (median) age of 60.
