The accident was notified to the Air Accidents Investigation Branch (AAIB) on 9 January 1998 at 1530 hrs and the investigation was initiated that afternoon.
In this configuration the upwards blast of the condenser exhaust air impinged on the underside of the floor panels, floor support structure and several wiring looms, and it was apparent that a proportion of this flow was re-circulated through the inlet of the condenser heat exchanger.
The worst case temperature of condenser discharge air is about 155°F however in the cruise it is usually in the region of 90°F. The blue painted cover for the chiller unit was made from a single sheet of aluminium folded to form the top and two sides of the box, and its attachment flanges.

Air Malta in a press release said that UK Air Accidents Investigation Branch (AAIB) report on the incident that occurred to Air Malta’s flight KM 197 (Glasgow-Malta) on the 1st September 2004 confirmed that Air Malta operates within the tyre manufacturer’s specifications and aviation industry standards.
Air Malta operates to European Joint Aviation Authorities (JAR) standards and is locally overseen by the Malta Department of Civil Aviation (DCA) and all engineering and flight operations practices must conform to State and International regulatory aviation standards.
Air Malta is also in the final stages of concluding the report on this and the other similar incident that occurred to flight KM572 (13th August 2004) flying to Moscow Domododevo.

An official accident report released last night revealed a fatigue crack had contributed to the collapse of the undercarriage as the plane touched down in May, 2001.
The Air Accidents Investigation Branch report on behalf of the Department of Transport confirmed this was the third case of a McDonnell Douglas MD-80 suffering landing gear failure due to fatigue cracking.
The AAIB is aware of a further case of MLG failure due to a similar mechanism, occurring to an aircraft of the same type with a Chinese operator in April 1997.

 Air Accidents Investigation Branch: History
It was established in 1915 when Captain C B Cockburn was appointed to the independent post of "Inspector of Accidents" for the RFC, reporting directly to the Director General of Military Aeronautics in the War Office.
At the end of the First World War a Department of Civil Aviation was set up in the Air Ministry and the AIB became part of that Department being concerned with investigation of both civil and military accidents.
The Air Navigation Act 1920 gave the Secretary of State for Air power to make regulations for the investigation of civil air accidents and the first regulations made for this purpose were the Air Navigation (Investigation of Accidents) Regulations 1922.

Air accident investigators are urging a review of air safety over parts of Britain after a series of near misses between civilian and military jets.
The AAIB report urges the Civil Aviation Authority to "re-examine the airspace categorisation, procedures and services currently available to Civil Air Transport aircraft...with the aim of ensuring that a level of protection is afforded to such aircraft from military aircraft".
It should be conducted "with the aim of eliminating airproxes [aircraft proximities] and potential collisions, with likely large scale loss of life," the AAIB said.
