On Aug. 21, 2004, an Access Air Ambulance flight carrying an 11-day-old infant and her mother to a Reno hospital slammed into a mountainside in a remote area of northern Nevada.
In 2000, the air ambulance trade group called on the FAA to push companies to emulate the type of training used by airlines to minimize mistakes.
Known at airlines as "Crew Resource Management," the training teaches pilots to listen to concerns from other crewmembers and to monitor themselves for factors such as fatigue and tension.

There were two cases to which I would like to refer where passengers died as they sat waiting for crew evacuation instructions, that were never given.
In both cases the cause of the accident was 'fire' and in both the pilots made successful landings.
The smoke was so thick in the cockpit the pilots were forced to look out the opened side cockpit windows to keep the wings level as they touched down.

Over 100 Troop-Carrier C-47s and crews had assembled at Foggia Air Field in southern Italy along with hundreds of fully-equipped paratroopers ordered to stand by for a drop.
Foggia was a B-17 base and so not to interfere with their daily bombing missions we were ordered to park our C-47s wingtip to wingtip not far off the grass crash strip which was parallel and next to the main runway.
