  Encyclopedia: Allergic reactions during anaesthesia
The frequency of life-threatening anaphylactic or anaphylactoid reactions occurring during anaesthesia has been estimated to be between 1 in 1000 and 1 in 25,000 anaesthetic procedures, with the neuromuscular blockers being involved in 80% of cases.
During anaesthesia, the clinical features of an allergic reaction are often masked.
Crawford Williamson Long was the first to use anaesthesia during an operation; giving it to a boy before excising a cyst from his neck, however, he did not publisize this information until later.

 Anaesthesia FAQ
Anaesthesia and surgery can affect all the systems of the body, therefore it is vitally important that the anaesthesiologist should be informed of all medical conditions you may have.
During some procedures the loss of blood is unavoidable, regardless the skills of the surgeon.
Allergic reactions to one type of anaesthetic does not mean that you will not be able to receive anaesthetics in future.

During the 48-h study period, median visual analog scale values of pain were approximately 10 mm at rest and from 18 to 25 mm during physiotherapy.
Supplementary analgesia during the intraoperative period was required by 45% of patients in the lidocaine group as compared with 9% in the mepivacaine group (P=0.02).

On the 16th of October 1846, another dentist, William Thomas Green Morton, invited to the Massachusetts General Hospital, performed the first public demonstration of sulfuric ether as an anesthetic agent, for a patient undergoing an excision of a tumour from his neck.
Discovered in 1831, its use in anaesthesia is usually linked to James Young Simpson, who, in a wide-ranging study of organic compounds, found chloroform's efficacy in 1847.

Although drug intolerance suggests an increased pharmacodynamic effect occurring among susceptible people, idiosyncratic and allergic reactions are unrelated to the amount of drug in the body and cannot be explained by an understanding of the normal pharmacodynamics of the drug given in usual therapeutic doses.
Allergic complications occurred in 7% of patients, and cardiovascular complications in 16%.
In 85 patients LaForest evaluated for reactions, 46% noted a history of allergy or atopy.63 In the first North American study, Moscicki and coworkers reported the incidence of atopy in 27 patients evaluated for anaphylaxis was 44.4%; the reported incidence of atopy in the U.S. population was 5% to 22%.

Determining the incidence of anaphylactic reactions during anesthesia or the intraoperative and postoperative periods is difficult because they are often attributed to toxic, pharmacologic, or anesthetic effects rather than allergy.
Reactions caused by mast cell–mediator release can be confused with other causes of hypotension or increased resistance to airflow, such as myocardial infarction; cardiac dysrhythmia; drug overdose; pulmonary embolus; irritant-induced bronchospasm; misplaced, blocked, or kinked endotracheal tube; pulmonary edema; aspiration of gastric contents or foreign body; seizure disorder; hypoglycemia; and stroke.
Reactions were attributed to the solubilizing agent, cremophor EL (polyoxyethylated castor oil), by means of complement activation and subsequent mast cell degranulation.

About 15% of all hospitalized patients will experience an adverse reaction to drug therapy, and it is estimated that truly allergic reactions occur in about 5% of all treatments (Ref.1).
A familial tendency for anaphylactoid reactions to drugs has been clearly identified in certain families although the mode of inheritance is unclear.
These reactions are due to cytotoxic antibodies binding to drug-hapten-tissue proteins and occur with the drug induced Coombs-positivehaemolytic anaemias and thrombocytopenias.

Use during anaesthesia: The possibility of prolonged or secondary apnoea should be considered if the drug is administered to anaesthetised patients who are concurrently receiving neuromuscular blocking agents such as succinylcholine, tubocurarine or decamethonium.
It is therefore advisable not to use gentamicin during pregnancy unless deemed essential by the physician and the expected benefits of therapy outweigh any potential risk.
Infrequent adverse reactions reported include purpura, convulsions, visual disturbances, leg cramps, increased serum transaminases, increased serum bilirubin, hypersensitivity reactions (rash, urticaria), nausea, vomiting, headache, leucopenia, granulocytopenia, transient agranulocytosis, increased and decreased reticulocyte counts and thrombocytopenia.

Allergic reactions can result from a true allergy to proteins occurring in natural rubber latex, or an allergy to one or more of the additional chemical substances used in the manufacture of the products, residual amounts of which will remain after processing.
Patients who have an allergy to latex are very likely to have allergic reactions to foods such as avocados, bananas, and chestnuts.
To minimise risk of anaphylactic reaction to latex, latex-free surgical gloves such as Elastyfree gloves, should be utilised during surgical procedures.
