Topic: Alosetron

  MedlinePlus Drug Information: Alosetron
Alosetron may cause serious gastrointestinal (GI; affecting the stomach or intestines) side effects including ischemic colitis (decreased blood flow to the bowels) and severe constipation that may need to be treated in a hospital and may rarely cause death.
Alosetron is used to treat diarrhea, pain, cramps, and the feeling of an urgent need to have bowel movements caused by irritable bowel syndrome (IBS; a condition that causes stomach pain, bloating, constipation, and diarrhea) in women who have diarrhea as their main symptom and have not been helped by other treatments.
Alosetron is in a class of medications called serotonin antagonists.

The 5-HT receptor antagonist alosetron has been assessed in patients with IBS, and dose-ranging studies showed that, in women, 1 mg alosetron twice daily was the most effective dose for improving IBS abdominal pain and discomfort, urgency, stool frequency, and stool consistency.
One patient in the alosetron group and two in the placebo group withdrew from the study before receiving the first dose and were not included in the safety analysis.
Alosetron significantly decreased urgency, stool frequency, and stool consistency, compared with placebo (where a decrease in stool consistency corresponds to an increase in stool firmness; figure 4).

DRUG INTERACTIONS: While alosetron is being broken down and eliminated by the liver, it may affect several enzymes in the liver that are responsible for the elimination of other drugs, and, therefore, it may increase or decrease the blood levels of these other drugs.
Other drugs also may alter the way alosetron is eliminated by the liver and increase or decrease blood levels of alosetron, but the effects of these drugs on alosetron have not been tested.

The dose of alosetron used in this Lancet study  (2 mg/day) is twice as much as the company is proposing (1 mg per day) in its new marketing plan being discussed at this meeting.
According to the FDA Medical Officer review of the use of this lower dose in dose-ranging studies prior to approval, there is no adequate evidence from placebo-controlled trials that the 1-mg per day dose is significantly better than a placebo.
We do not think that the necessary combination of safeguards to reduce the risk from alosetron can be accomplished under conditions of marketing; it needs to be done under a restricted IND. One of the ways being considered is the so-called Subpart H section of FDA regulations.

Alosetron is used to treat severe irritable bowel syndrome (IBS) in some women who have failed to respond to other treatments for severe diarrhea.
After alosetron was first approved by the U.S. Food and Drug Administration (FDA) for treatment of IBS, some people taking the medication suffered from serious side effects, including severe constipation or reduced blood flow to the large intestine (ischemic colitis).

Since drug safety must always be viewed in the context of drug benefit, a review of the evidence of benefit for alosetron in the treatment of diarrhea-predominant irritable bowel syndrome is appropriate.
Assuming the accuracy of the GSK estimate of 9,365 patients getting alosetron during the RMP, the rate of ischemic colitis reports (8/10,000 patients or 0.85 per 1000) is actually higher than the rate of spontaneous reports during the earlier 2000 marketing,   before market withdrawal (85/275000 patients or 0.31 per 1000).
But alosetron joins an increasing number of other drugs, none with unique, clinically significant benefits, that have been the subject of ultimately failed FDA-approved risk management programs (the diabetes drug Rezulin, the pain-killer Duract, the gastrointestinal drug Cisapride, the blood pressure drug Posicor) and were taken off the market.

NEW YORK, N.Y. -- October 16, 2000 -- The 5-HT3 receptor antagonist, alosetron, decreases time lost from work in women with irritable bowel syndrome whose predominant symptom is diarrhea, new results suggest.
The data were presented at the 65th Annual Scientific Meeting of the American College of Gastroenterology, in New York, and are based on the results of two trials in 903 women 18 years of age and older with diarrhea-predominant irritable bowel syndrome (IBS) and a normal gastrointestinal anatomy.
