This problem was first identified in 1972, when American Airlines Flight 96 lost its aft cargo door after takeoff from Detroit; fortunately the crew were able to perform an emergency landing with no further incident.
The United States National Transportation Safety Board officials discovered that a maintenance procedure was the culprit: American Airlines mechanics had removed the engine and its pylon at the same time using a forklift, and the forklift operator had inadvertently cracked the pylon in the process.
After the hydraulics failed on Flight 232, the pilots were able to crash-land the plane; although many died, over half of the passengers walked away without major injury.

We are Not saying, that was was NO plane, Flight 77 that crashed into the Pentagon.
This picture shows two vehicles on Route 27 that were damaged by light pole debris, clipped by the Flight 77 aircraft as it passed overhead and across Route 27, then plowed into the Pentagon.
The pole to the left is one of several that were clipped by the aircraft.

Pilots may also request clarification or amendment, as appropriate, any time a clearance is not fully understood, or considered unacceptable because of safety of flight.
