On the afternoon of Friday, May 25, 1979 (Memorial Day weekend), American Airlines Flight 191, a Los Angeles-bound DC-10, began its takeoff roll at 3:02:48 PM CDT from Chicago-O'Hare International airport.
Flight 191's left wing was pushed down by a stall which resulted when the slats (which extend the wing's leading edge during takeoff, increasing its lift and reducing its stall speed) on the outer part of that wing were pushed back in by wind pressure.
This happened after the hydraulic pressure which held the slats out during takeoff was lost as a result of the damage from the pylon separation (DC-10s were later fitted with mechanical locking devices to hold the slats out).

I think continental, and american airlines, and another airline came under fire for "cutting corners" in the way the pylon was installed.
Three airlines decided to ignore that recommendation and instead, removed the engine and pylon as one unit, in the interest of improved maintenance efficiency: United, American and Continental.
Because the forklifts were far less precise in their ability to mate the engine/pylon back to the wing, without causing impact damage to the attach point structures, both CAL and AMR suffered impact damage to the pylon attach point structures of their wings.

I worked for Eastern years after flight 401 went down in the everglades in Florida, and I heard countless tales (though under the table) of the sightings of co-pilot Don Remo, who died in the crash and returned to give warnings to crew members on future flights of immenent danger.
American 191 brought new pylon reinforcements for the engines on most aircraft.
I also got some good video of American Airlines gate K-5, which I'm told is the gate that serviced flight 191 - and yes, the infamous payphone bank where the man in the outdated business attire has disapeared.

But Philip Hogue, a member of the National Transportation Safety Board investigating the American crash, said that he thought the planes should have been kept in hangars until the cause of the disaster had been more fully determined.
The probe into the American crash seems certain to lead to a broader and deeper investigation, going all the way back to the initial design of the DC-10 by McDonnell Douglas Corp. and its certification by the FAA.
