Amikacin - Wikipedia, the free encyclopedia
Amikacin works by binding to the bacterial 30S ribosomal subunit, causing misreading of mRNA and leaving the bacterium unable to synthesize proteins vital to its growth.
Amikacin may be administered once or twice a day but must be given by the intravenous or intramuscular route.
Amikacin may be combined with a beta-lactam antibiotic for empiric therapy for people with neutropenia and fever.

 Amikacin
Amikacin is indicated for the treatment of infections of: central nervous system, urogenital system, biliary and intestinal tracts, skin and subcutaneous tissues, intraabdominal infections, pneumonia, caused by Gram-negative microorganisms, secondary infections after combustion, bacterial septicemia, infections of the bones and joints (caused by sensitive to Amikacin microorganisms).
In adults and children with normal renal function, Amikacin is administered intramuscularly as an intravenous bolus or by slow intravenous infusion in a dose of 5 mg/kg body weight given every 8 hours or 7.5 mg/kg body weight every 12 hours for a period of 7 to 10 days.
Amikacin has a bactericidal action which is due to the inhibition of the protein synthesis in the cells of the microorganisms.

 Amikacin Sulfate
1.3 grams of amikacin sulfate is equivalent to 1 gram of amikacin.
While amikacin is less susceptible to this effect, it is usually recommended to avoid mixing these compounds together in the same syringe or IV bag, unless administration occurs promptly.
Since amikacin is less affected by this effect than either tobramycin or gentamicin, it is assumed that reduction in serum levels will also be minimized using this procedure.

Renal and eighth-nerve function should be closely monitored especially in patients with known or suspected renal impairment at the onset of therapy and also in those whose renal function is initially normal but who develop signs of renal dysfunction during therapy.
The concurrent use of amikacin with potent diuretics (ethacrynic acid, or furosemide) should be avoided since diuretics by themselves may cause ototoxicity.

Because amikacin is the only currently available aminoglycoside effective against many gentamicin- and tobramycin-resistant organisms, many infectious disease experts have felt that its use should be restricted to minimize the emergence of additional resistant strains.
Amikacin is cleared by the kidney and accumulates in renal tubular cells.

Without a change in antibiotics, the response rates were 81 percent with ceftazidime and long-course amikacin, 48 percent with ceftazidime and short-course amikacin (P = 0.002), and 40 percent with azlocillin and long-course amikacin (P less than 0.001).
Linear logistic-regression analysis showed that therapy with ceftazidime and long-course amikacin was the most favorable prognostic factor of the response to infection, whereas the presence of leukemia or shock was the least favorable.
We conclude that ceftazidime should be given in combination with a conventional full course of an aminoglycoside (amikacin) when used for the empirical treatment of gram-negative bacteremia in cancer patients with granulocytopenia.
