Amiodarone   (Site not responding. Last check: 2007-11-07)
Amiodarone is a class III antiarrhythmic agent used in the treatment of ventricular arrhythmias and the suppression of atrial and ventricular arrhythmias.
Amiodarone shows beta blocker-like and calcium channel blocker-like actions on the SA and AV nodes, increases the refractory period via sodium- and potassium-channel effects, and slows intra-cardiac conduction of the cardiac action potential, via sodium-channel effects.
Amiodarone is structurally similar to thyroxine (a thyroid hormone), which contributes to the effects of amiodarone on thyroid function.

Amiodarone was approved by the FDA for clinical use as an antiarrhythmic agent in December of 1985.
Amiodarone is considered a "broad spectrum" antiarrhythmic medication, that is, it has multiple and complex effects on the electrical activity of the heart which is responsible for the heart's rhythm.
Amiodarone increases the blood levels of digoxin (Lanoxin) when the two drugs are given together.

Ophthalmic Hyperguide. Neuro-ophthalmology: Amiodarone Optic Neuropathy
In contrast to amiodarone keratopathy, the data on amiodarone optic neuropathy are less convincing for causality using the Hill criteria.
The researchers postulated that amiodarone may have a chronic neurotoxic effect on the optic nerve via a drug-induced lipidosis, but the linkage to a clinical presentation of anterior ischemic optic neuropathy was not defined.
Amiodarone keratopathy is temporally associated with drug onset, a dose-dependent effect seen in the majority of patients on the drug, produces a characteristic and specific corneal appearance that cannot be attributed to alternative etiologies, has been pathologically proven to be due to drug deposition in the epithelium, and typically resolves after discontinuation of the drug.

Amiodarone: From Last to First-Line Antiarrhythmic Therapy
Although amiodarone therapy typically is initiated in the hospital under the direction of a cardiologist, long-term amiodarone monitoring and evaluation often become the responsibility of the primary care physician.
Amiodarone discontinuation, if therapeutically possible, may be attempted, although hyperthyroid symptoms and cardiac status may worsen as the antiadrenergic effects of the drug wear off.
Proarrhythmia secondary to amiodarone therapy is a concern because of the drug's propensity to markedly prolong the QT interval with a potential for torsade de pointes.

ACUTE CARE, INC. - Emergency Physicians - Practice Opportunities, Information
The administration of 300mg amiodarone (Cordarone) in cardiac arrest patients experiencing ventricular fibrillation, is creating compelling evidence, in the minds of many clinicians, that challenges previously identified antidysrythmics (lidocaine, procainamide, bretylium).
While amiodarone was well-tolerated in this critically ill population, there was a higher incidence of hypotension and of bradycardia requiring therapy in the amiodarone group.
Amiodarone is primarily a class III antiarrythmic that prolongs duration of action potential and effective refractory period, also providing noncompetitive alpha and beta adrenergic inhibition.

Amiodarone to prevent a-fib after heart surgery
It´s well known that Amiodarone has a high content of Iodine in its molecule and, in a high percentage of patients it may provoke clinical and/or subclinical changes on respect to thyroid gland function such as hyperthyroidism or mild changes in TSH or Thyroxine.
Amiodarone has been very questioned among cardiologists because of its benefit/risk ratio and also because there are the other therapies such as Propafenone Clorhidrate that might have similar benefits at lower risk.
Thyroid function studies after amiodarone therapy would have been interesting, although the number of patients studied here was probably too small to reliably exclude any alteration in thyroid function occurring in a small number of patients.

Patients were begun on a regimen of intravenous amiodarone because of frequent sustained ventricular tachycardia unresponsive to conventional therapy with intravenous antiarrhythmic medications.

CHEST: Is Amiodarone an Underrecognized Cause of Acute Respiratory Failure in the ICU? - )   (Site not responding. Last check: 2007-11-07)
Amiodarone is a commonly used anti-arrhythmic agent, with well-recognized chronic toxicity.
Amiodarone is the one of the most commonly prescribed anti-arrhythmic agents worldwide.[1] Although initially developed as a new class of antianginal vasodilator, its derivative, desethylamiodarone, proved to be a powerful antiarrhythmic agent with atypical class III Vaughan-Williams properties, provoking substantial clinical interest.
Thus, despite amiodarone's intended therapeutic target being the heart, amiodarone concentrations measured in unfractionated pulmonary parenchyma significantly exceed that of the heart.[7] Accumulated concentrations of amiodarone's derivative in particular lung compartments, such as the type II pneumocyte, further exaggerate this effect.

Ventricular Tachycardia After Amiodarone: Report of an Unusual Case
Amiodarone has been approved for treatment of patients with ventricular and supraventricular tachyarrhythmias.1—3 Previous reports have found that it is effective for the conversion of atrial fibrillation and subsequent maintenance of sinus rhythm.2—6 In this report, we describe a case of atrial fibrillation that converted with amiodarone.
The amiodarone dose was changed to 200 mg daily for 5 days, but 400 mg twice a week for 4 weeks, that is, the patient received 1,800 mg amiodarone weekly.
Although low-dose amiodarone reduces the occurrence of atrial fibrillation in patients in which therapy is indicated, it can potentially cause serious ventricular arrhythmias, as was seen in the patient presented in this case report.

Data presented at the American Heart Association's (AHA) Scientific Sessions 2001 in Anaheim, Calif., demonstrated that the antiarrythmic agent IV amiodarone (marketed as Cordarone I.V.) is almost twice as effective as lidocaine in keeping cardiac arrest patients alive until they reach a hospital.
The trial found that 22.8% of cardiac arrest patients treated with IV amiodarone survived to hospital admission compared with 12% of patients treated with lidocaine.
Dosages: Amiodarone IV was administered as a bolus dose of 5 mg/kg, whereas 1.5 mg/kg of lidocaine was administered.
