CiteULike: Colon and anal sphincter contractions evoked by microstimulation of the sacral spinal cord in cats.
The colon and anal sphincter contractions induced by microstimulation of the S2 spinal cord were investigated by measuring the intraluminal pressure change via saline filled balloons in alpha-chlorolose anesthetized cats.
Locations in S2 spinal cord, where microstimulation evoked a distal/proximal colon pressure response that was greater than the anal sphincter response, included the area of sacral parasympathetic nucleus (SPN), the area medial to the SPN extending to the dorsal commissure, and areas deep in the ventral horn.
Anal sphincter relaxation was evoked by microstimulation in more restricted locations in S2 spinal cord, which appeared to overlap with those evoking anal sphincter contractions.

University Surgeons - Artificial Anal Sphincter
Anal sphincter: a series of circular muscles, the majority of which are under voluntary control, which prevent the release of gas or stool from the anus.
The injury to the anal sphincter is identified and the injured muscle edges are freed from surrounding scar tissue.
The artificial anal sphincter is a totally implantable, simple-to-use device consisting of three components: 1) an inflatable cuff; 2) a fluid reservoir (balloon); and 3) a semi-automatic pump connected between the cuff and balloon.

The internal anal sphincter is a thin, white muscle wrapped around the anal canal.
The internal sphincter contracts during rest and sleep, and keeps small amounts of liquid and gas from escaping unexpectedly.
Anal stenosis means that the anal opening is too small, or that it does not relax or open properly.

Anal fissure (torn rectum) - causes and treatment options at MedicineNet.com
TheĀ goal of treatment for anal fissures is to break the cycle of spasm of the anal sphincter and its repeated tearing of the anoderm.
Anal fissures are diagnosed and evaluated by visual inspection of the anus and anal canal.
Anal fissures are initially treated conservatively by adding bulk to the stool, softening the stool, consuming a high fiber diet, avoiding “sharp” or poorly digested foods, and utilizing sitz baths.

 Anal Fissure
An anal fissure is a tear in the lining of the lower rectum (anus) that causes pain during bowel movements.
Anal fissures are caused by vigorous stretching of the anal canal.
In some cases, an anal fissure may be caused by Crohn's disease, an inflammatory bowel disease (IBD) that causes bloody diarrhea, abdominal pain, fever, weight loss, and fissures or fistulas near the anus.

Below the pectinate line, an anal intersphincteric groove is palpable between the subcutaneous part of the external sphincter and the lower border of the internal sphincter.

The anal stretch procedure is rarely done as there is a high risk of faecal incontinence and tears of the anal sphincter.

Anal fissure, cut in anus area, anal pain
An anal fissure is a small tear or cut in the skin around the anus which can cause pain and/or bleeding.
A chronic fissure may be due to poor bowel habits, tight or spastic anal sphincter muscles, scarring or an underlying medical problem.
Historically chronic fissures were treated with surgical disruption or division of the internal anal sphincter muscle.

 Treating the Anal Sphincter
There is evidence that hemorrhoid sufferers have increased tightness of the anal sphincter, and some studies are showing that a variety of anal sphincter-relaxing remedies may promote improvement in prolapsed internal hemorrhoids and thrombosed external hemorrhoids, possibly avoiding surgery, or make it possible to to use techniques short of scalpel hemorrhoidectomy.
ANAL FISSURES have been found to be associated with increased anal sphincter muscle tone, and the research demonstrating fissure healing with calcium channel blockers and other agents may be applicable to external hemorrhoids......

Palo Alto Medical Foundation General Surgery Center: ANAL FISSURE -- BOTOX
An anal fissure, or split in the skin of the distal anal canal, is a common problem that causes substantial morbidity in people who are otherwise healthy.
The majority of fissures occur in the posterior midline of the anal canal and are usually the result of tearing of the skin from hard stool or other physical irritants.
Spasm of the anal sphincter has been noted in association with anal fissure, and for many years treatment has focused on alleviating spasm of the sphincter.

 Anal atresia
In boys with high anal atresia, there may be a channel (fistula) connecting the large intestine to either the urethra (which delivers urine from the bladder) or the bladder itself.
Anal atresia is a defect in the development of the fetus.
For high anal atresia, immediately after the diagnosis is made, a surgical incision is made in the large intestine to make a temporary opening (colostomy) in the abdomen where waste is excreted.

eMedicine - Anal Fissure : Article Excerpt by: Lisa S Poritz, MD
Problem: An anal fissure is a painful linear tear or crack in the distal anal canal, which, in the short term, usually involves only the epithelium and, in the long term, involves the full thickness of the anal mucosa (see Images 1-2).
Studies of the internal anal sphincter and of anal canal physiology have been performed with varied results, but at least one abnormality is likely present in the internal anal sphincter of many anal fissure patients.
In patients with hypertrophied internal anal sphincters, this delicate blood supply is further compromised, thus rendering the posterior midline of the anal canal relatively ischemic.

 UpToDate Patient information: Anal fissure
There are no reliable estimates of the frequency of anal fissures in the general population; some studies suggest that as many as one in five persons develop a fissure during their lifetime.
Anal fissures are caused most frequently by trauma that stretches the anal canal, such as after passage of large or hard bowel movement or explosive diarrhea.
The goal of lateral sphincterotomy is to relax the internal anal sphincter by cutting a small nick into the internal anal sphincter of the anal canal.

The PAS Prosthetic Anal Sphincter device has been designed and developed over a period of 10 years to manage the condition of faecal incontinence based on studies of the normal function of the anal sphincter and pelvic floor.
The PAS device is designed for the management of severe faecal incontinence in adult men and women, and aims to overcome the risk of ischaemic complications by simulating the normal physiology of the ano-rectum and reproducing the action of the puborectalis muscle by flattening and angulating the bowel without causing crenation.
Comprises of an all-in-one design of a textured silicone cohesive gel-filled soft pillow that is attached by silicone elastomer straps to an inflatable linear expander, which when inflated, gently flattens the bowel against the silicone cohesive gel-filled pillow.
