Analgesic - Wikipedia, the free encyclopedia
An analgesic (colloquially known as a painkiller) is any member of the diverse group of drugs used to relieve pain and to achieve analgesia.
Analgesic drugs act in various ways on the peripheral and central nervous system; they include paracetamol (acetaminophen), the nonsteroidal anti-inflammatory drugs (NSAIDs) such as the salicylates, narcotic drugs such as morphine, synthetic drugs with narcotic properties such as tramadol, and various others.
Analgesics are frequently used in combination, such as the paracetamol and codeine preparations found in many non-prescription pain relievers.

And, the painkillers that combine two or more analgesics (for example, aspirin and acetaminophen together) with caffeine or codeine are the most likely to damage the kidneys.

Analgesic Nephropathy (Painkillers and the Kidneys)   (Site not responding. Last check: 2007-09-16)
Analgesic nephropathy is a chronic kidney disease that over years gradually leads to irreversible kidney failure and the permanent need for dialysis or a kidney transplant to restore renal function.
Longstanding daily use of painkillers composed of two or more analgesics (particularly aspirin and acetaminophen together) with caffeine or codeine are most likely to damage the kidneys.
Recent studies have suggested that longstanding daily use of single analgesics such as acetaminophen or ibuprofen may also increase the risk of chronic kidney damage, but this evidence is not as clear.

The worldwide analgesic market is estimated to be worth $50 billion during the year 2005 and is expected to increase to $75 billion by the year 2010 and $105 billion by the year 2015..
Since use of analgesics is widespread and new OTC products are introduced frequently, the potential impact of these drugs on the development of chronic renal failure may be significant, thus warranting continued evaluation of these products for any renal toxicity.

Methadone: an essential analgesic to manage pain in cancer
Professor Eduardo Bruera, an internationally respected expert in clinical oncology and palliative medicine, reviews the clinical merits of methadone, the advantages and challenges of switching from other opioids to methadone, and the appropriateness of using methadone as a first-line opioid for cancer pain.
The second barrier is the stigma attached to methadone in the mind of some physicians who have not distinguished methadone's role in pain management from its role as a maintenance therapy for opioid dependence.
Analgesics with opioid-antagonist properties (e.g., buprenorphine, butorphanol, nalbuphine) should not be used with methadone because they can displace methadone from Mu-opioid receptors.

Improving Analgesic Drug Development: What's Needed
The limitations of currently available analgesic drugs often result in failure to alleviate suffering in patients with chronic diseases and cancer, patients at the end of life, and in vulnerable populations.
The expense and uncertainty of analgesic drug development also present barriers to the development of novel drug therapies to improve pain management, much less to prevent the development of chronic pain.
Yet, the quest continues for analgesic drugs with a unitary mechanism of action to minimize adverse effects and to be used by the largest possible population.
