Morton himself seems to presume too much on his knowledge of Madonna: he constantly claims that she was miserable, depressed, et cetera.
She is often portrayed as a heartless climber, but Morton paints the picture of a bright personality who is extremely energetic, motivated, and also loving and caring, even if she does crave the spotlight and need to be the center of attention.
Morton is a fan of Maddy's, as he treats her with relative kid gloves, as opposed to some other recent biographies.

The feverish efforts to rush the book into print, the “secrecy” behind the project, the tie-in with the Barbara Walters interview and the desperate sincerity of Morton to convince us (and himself) that his subject was more that what she really is left them wide open for gags about greedy mercenaries and empty-headed bimbos.
As relayed by Lewinsky through Morton, the president is a bland, occasional presence whose memories of his life sound straight out of a campaign biography.

A co-writer usually gets public credit and equal billing with the other writer or writers (in terms of type size on the cover, in the ads, etc).
In your case, the project is an "as told to" with the newspaper writer.
With "as told to's," the person telling the story is a celebrity or newsworthy person who is not a professional writer, and therefore is telling the story to the professional, who will actually be whipping the mass of interviews, archive material, notes, etc. into a publishable book.
