Topic: Ankylosis

Ankylosis - Wikipedia, the free encyclopedia
Ankylosis, or Anchylosis (from Greek αγκυλος, bent, crooked) is a stiffness of a joint, the result of injury or disease.
The rigidity may be complete or partial and may be due to inflammation of the tendinous or muscular structures outside the joint or of the tissues of the joint itself.
When the structures outside the joint are affected, the term "false" ankylosis has been used in contradistinction to "true" ankylosis, in which the disease is within the joint.

Ankylosis of the Canine Carpal Joints in a dog
Ankylosis, as a medical condition, is the complete loss of movement in a joint caused by degeneration and fusion of the bony surfaces.
Ankylosis may also occur as a consequence of surgical fusion of a diseased joint to correct a deformity or to alleviate persistent pain (4, 8).
Symptoms of ankylosis may be temporarily relieved by supporting the joint with a splint or cast, the use of analgesics, nonsteroidal anti-inflammatory drugs, or occasionally, an injection of corticosteroid drugs.

THE MERCK MANUAL, Sec. 9, Ch. 108, Temporomandibular Disorders
Ankylosis of the TMJ most often results from trauma or infection, although it may accompany RA or be congenital.
Intra-articular (true) ankylosis must be distinguished from extra-articular (false) ankylosis, which may be caused by enlargement of the coronoid process, depressed fracture of the zygomatic arch, or scarring from surgery, irradiation, or infection.
Treatment may include a condylectomy if the ankylosis is intra-articular or an ostectomy of part of the ramus if the coronoid process and zygomatic arch are also affected.

Brazilian Oral Research - Tooth ankylosis in deciduous teeth of children with cleft lip and/or palate   (Site not responding. Last check: 2007-10-31)
The aim of this study was to characterize the prevalence of ankylosis in deciduous molars of individuals with cleft lip and/or palate due to the importance of early diagnosis of this anomaly to prevent the development of malocclusions, since these patients already present changes in anatomy and tooth position, which are inherent to their malformation.
The data related to the prevalence of tooth ankylosis according to the type of cleft and the most affected arch and teeth are displayed in Tables 1 and 2, respectively.
No statistical difference between the occurrence of ankylosis and the different types of clefts (Table 1) could be found, suggesting that, even though clefts may bring about distinct dental alterations in the maxilla, according to the diversity of its anatomical involvement, they do not significantly interfere with the occurrence of infraocclusion.

The determination will be made on the basis of the actual remaining function of the hand or foot, whether the acts of grasping, manipulation, etc., in the case of the hand, or of balance and propulsion, etc., in the case of the foot, could be accomplished equally well by an amputation stump with prosthesis.
This 10 percent rating and the other partial ratings of 30 percent or less are to be combined with ratings for ankylosis, limited motion, nonunion or malunion, shortening, etc., subject, of course, to the amputation rule.
Where, however, the limitation of motion of the specific joint or joints involved is noncompensable under the codes a rating of 10 percent is for application for each such major joint or group of minor joints affected by limitation of motion, to be combined, not added under diagnostic code 5002.

Temporomandibular joint (TMJ) ankylosis is a structural disease that can cause asymmetry resulting in severe facial disfigurement as well as difficulties in eating, breathing, and speech.
True ankylosis is a condition that results in osseous of fibrous adhesion between the surfaces of the TMJ, within the limits of the articular capsule.
Management of TMJ ankylosis is through surgical intervention as soon as the condition is recognized.

The differentiation of cases of ankylosis is an important one.
Da Costa says that a joint immovable from fibrous ankylosis is distinguished from a joint immovable from bony ankylosis by the fact that, in the former, attempts at motion are productive of pain and subsequently of inflammation; therefore, pain on attempted motion excludes bony ankylosis from our diagnosis.
For bony ankylosis nothing should be attempted, for the treatment would only result in discouragement and disappointment to both physician and patient; but if the joint is in an almost useless position, excision or osteotomy may be tried with good results.

Temporomandibular joint (TMJ) ankylosis is characterized by the formation of bony or fibrous mass, which replaces the normal articulation and limitation of mouth opening.
The term temporomandibular (TM) ankylosis refers to bone or fibrous adhesion of the anatomic joint components and the ensuing loss of their function.1 When the career of the patient is considered, TM joint (TMJ) ankylosis is a very distressing structural condition that alters the patient's eating habits and speech ability.
Management of TMJ ankylosis is mainly performed through surgical intervention.2 Various techniques for the management of TM ankylosis have been described.

Bony ankylosis of the temporomandibular joint (TMJ) in a male patient was not diagnosed until the patient reached his early teens, at which time the condition was treated with a costochondral graft.
However, with the release of the ankylosis and growth of the costochondral graft, a good functional and esthetic result was achieved without further surgery.
Mandibular hypomobility resulting from TMJ ankylosis is classified according to location (intracapsular or extracapsular), type of tissue involved (bony, fibrous or fibro-osseous) and extent of fusion (complete or incomplete).

This study was done to evaluate the incidence of TMJ ankylosis, its etiology, effects of its duration on facial aesthetics, types and stages of ankylosis observed during operation.
With in the limitations of this study it is concluded that management of temporomandib ularjoint ankylosis is difficult particularly in long-standing bilateral cases with severe facial deformity.
It is suggested that in all such cases where gap produced after removing of ankylosis, is much larger and ramus length become short, should be corrected by, grafting with costochondral part of rib along with interposition by silastic or temporal muscle flap.

Joint ankylosis is the hallmark, as the name less than subtlely suggests.
Ankylosis of the posterosuperior ligamentous portion is considered to look like a "star." The first CT above gave you an idea of the initial involvement of the anteroinferior SI joint portion, while the next CT shows the more diffuse involvement at a later stage.
Other findings you might see in the pelvis, if you're not overwhelmed by those SI joints and a glimpse of the spine, are ossification of ligamentous attachments in the iliac crests and ischial tuberosities, classically giving a purported "whiskered" look.

Effect of high-dose hydrocortisone treatment on inflammation and intraarticular calcium hydroxyapatite deposits. When affected (ank/ank) mice were treated with high doses of hydrocortisone, synovitis receded, development of cartilaginous and bony osteophytes halted, and calcium hydroxyapatite accumulated in and distended the synovial spaces.
Since inhibition of inflammation by hydrocortisone treatment did not block apatite accumulation, intraarticular deposition of hydroxyapatite occurs independent of inflammation in progressive ankylosis.
Mice homozygous for the progressive ankylosis trait develop an inflammatory joint disorder associated with the intraarticular deposition of calcium hydroxyapatite.

Progressive ankylosis (ank), a murine autosomal recessive mutation, produces an inflammatory joint disorder associated with intraarticular calcium hydroxyapatite deposition and culminates in fusion of the joints.
Subsequently, cartilaginous osteophytes, extending outward from the subchondral bone, bridge the margins of the joint and undergo ossification.
The progressive ankylosis mutation provides a useful system for investigating calcium hydroxyapatite-associated arthropathies.
