Topic: Anterior chamber

eMedicine - Postoperative Flat Anterior Chamber : Article by James V Aquavella, MD
The anterior chamber is deepest (approximately 3 mm) in its central portion and shallowest at the peripheral insertion of the iris.
The depth of the anterior chamber is estimated as the distance between the posterior surface of the cornea and the front surface of the iris.
The depth of the anterior chamber is an important indicator of the integrity of the postoperative globe and may be useful in the differential diagnosis of existent pathology.

Inflatable vaginal pessary - Patent 5611768
Thus the anterior chamber of a pessary of the invention can extend into the fornix region or the anchor member (posterior chamber) can extend into the vestibule region, as long as the other essential features of the invention are present in the device.
If multiple chambers are used in either the fornix or the vestibule, the aggregate lengths, cross sectional dimensions and volumes of those chambers should approximate the lengths, cross sectional dimensions and volumes disclosed for posterior and anterior chambers, respectively, of the two-chambered pessary.
The "tube" thus represents whatever shape is present to connect the chamber to the valve and may merely be the end of the chamber itself (e.g., for the anterior chamber that is adjacent to the external opening of the vagina) or an extension thereof.

In the anterior chamber, if you have a prolene suture through the iris, I think you have to admit that it would be subject to more light degradation than a suture that is buried under a scleral flap.
If I felt that an anterior chamber lens was the most appropriate for the patient at the time of surgery, and I didn't wish to extend the 3-mm temporal clear corneal incision, I would move superiorly, open the conjunctiva and make a sclero-corneal tunnel for lens implantation at the same sitting.
Studies indicate that a well-positioned anterior chamber lens is tolerated by the graft.

The source of anterior chamber hemorrhage in contusion injuries is believed to be related to the displacement of tissues posteriorly or to the resultant fluid wave in the aqueous and vitreous, which stretches the limbal vessels and displaces the iris and lens.
To avoid the iris and lens, the blade is oriented and pushed into the anterior chamber in such a manner that it is parallel to the plane of the iris.

According to the surgeon, ECCE was attempted because of an extraordinarily dense nuclear cataract and a shallow anterior chamber; he sensed that phacoemulsification was too risky with respect to the cornea.
Through a beveled paracentesis, the anterior chamber is deepened with a dispersive viscoelastic material and posterior synechias are gently lysed with a cyclodialysis spatula.
The anterior chamber is then reformed with an intraocular miotic, and the pupil is mechanically reduced using a Lester hook.

Surgical contamination of the anterior chamber during lens surgery
Anterior chamber aspirates collected at the conclusion of surgery were culture positive for at least one organism in 22.7% of eyes.
Two fungi and one bacterium isolated from the anterior chamber were typed identically, and the bacterium had a similar antibiogram to organisms recovered from the patient`s conjunctiva and eyelid margin.
We conclude that intraoperative contamination of the anterior chamber with viable bacterial and fungal organisms is a common occurrence in canine patients undergoing cataract phacoemulsification and intraocular lens implantation, and the external ocular flora is a likely source of some of these contaminating microorganisms.
