Anti-thymocyte globulin - Wikipedia, the free encyclopedia
Anti-thymocyte globulin (ATG) is an infusion of rabbit-derived antibodies against human T cells which is used in the prevention and treatment of acute rejection in organ transplantation and therapy of aplastic anemia.
Anti-IL-2Rα receptor antibodies such as basiliximab and daclizumab are increasingly being used in place of ATG as an induction therapy, as they do not cause cytokine release syndrome and (theoretically) improving the development of tolerance.
ATG is associated with cytokine release syndrome when it is used, and in the longer term may increase the risk of post-transplant lymphoproliferative disorder as well as (theoretically) impairing the development of immunological tolerance.

 AA&MDSIF - Glossary Terms
ATG (anti-thymocyte globulin) or ALG (anti-lymphocyte globulin) are immunosuppressive agents used for the treatment of aplastic anemia patients.
ATG is made by immunizing horses with thymic lymphocytes (thymocytes) obtained from the thymus or from the thoracic duct.
ATG is given into a vein (intravenous, IV) while the patient is in the hospital, usually over four days (4-24 hours a day).

 AA&MDSIF - Immunosuppressive Therapy
ATG and cyclosoprine, which are standard in the treatment of aplastic anemia, are classified as pregnancy category C drugs, which means that they can be used when needed during pregnancy though their safety has not been officially proven.
However, if the drug together with ATG were to be responsible for the long-term survival of a whole group of patients who would not have otherwise survived, then it might have the appearance of increasing the number of cases of progression to clonal disease, simply by increasing the number of patients at risk.
Cyclosporine and ATG are the standard of care for severe aplastic anemia.

Since the anti-adhesive activity in vitro correlates with graft survival in vivo it is an attractive explanation for the prolonged immunosuppression.
Compared to therapeutic approaches using monoclonal antibodies against single adhesion molecules the multiple specificities of ATGs may be of additional benefit.
Thereby the adhesion cascade can be interrupted at the activation step even a considerable time after the application of the drug when the ATG content in plasma is levelling off.

Anti-thymocyte globulin (equine- and rabbit-sourced) is used to reduce the immune response in patients who have received skin, cardiac, liver, or bone-marrow transplants.
Anti-thymocyte globulin will be administered as an adjunct to other immunosuppressant therapy to delay the onset of the first rejection episode of a transplant.
Because of this, the safe use of anti-thymocyte globulin requires individualized instructions and direct practitioner supervision.

Anti-thymocyte globulin is strongly reactive with platelets as measured by flow cytometric analysis.
This reactivity appears to be independent of human leukocyte antigen (HLA) phenotype, is dose-dependent with saturation of platelet binding sites readily achieved with concentrations of horse anti-thymocyte globulin (ATG) at 0.25 mg per ml, and cannot be blocked with human anti-PLA1 antibody nor with heat-aggregated human IgG.
