Topic: Aortic arch interruption

Interrupted Aortic Arch / Ventricular Septal Defect, Cincinnati Children's Hospital Medical Center
Interrupted Aortic Arch (IAA) is the absence or discontinuation of a portion of the aortic arch.
Interrupted Aortic Arch is thought to be a result of faulty development of the aortic arch system during the fifth to seventh week of fetal development.
In patients with Interrupted Aortic Arch, oxygen-rich blood from the left side of the heart is not able to reach all areas of the body because of the defect in the aortic arch.

A diagnosis of interrupted aortic arch type B was made and termination of the pregnancy was offered due to the poor prognosis associated with a hydropic fetus at 23 weeks gestation.

Coarctation, pseudocoarctation, interruption of the arch aortic, aberrant ramification with or without vascular rings, are some of the anomalies of the aortic arch whose diagnosis is possible by means of the radiological studies, magnetic resonance imaging (MRI) and spiral computed tomography (CT).
Achiron et al., indicate that the anomalies of the aortic arch require a clear definition of the position of the fetal trachea in relation to the pulmonary artery, the ductus arteriosus and the aortic arch.
Aortic diverticulum or Kommerell’s diverticulum is a rare aortic congenital anomaly that is produced as a result of the aberrant origin of the left subclavian artery, that arises of right arch aortic as a fourth branches and passes behind of the esophagus to the left shoulder instead of arising from the right brachiocephalic artery.

Background: Interrupted aortic arch (IAA) is a relatively rare genetic disorder that occurs in association with a usually nonrestrictive ventricular septal defect and ductus arteriosus or, less commonly, with a large aortopulmonary window or truncus arteriosus.
In type B interrupted left aortic arch, the interruption occurs distal to the origin of the left common carotid artery.
In type C interrupted left aortic arch, the interruption occurs proximal to the origin of the left common carotid artery.

Aortic coarctation is most likely related to an abnormality in the pattern of ductus arteriosus blood flow in utero, which, in turn, may be the result of associated intracardiac anomalies.
The dependence of aortic obstruction on constriction of the ductus arteriosus postnatally explains the variable onset after birth of the clinical manifestations of coarctation, as well as the dramatic alleviation of the obstruction produced pharmacologically by dilatation of the ductus arteriosus.
A spectrum of anatomic lesions exists, causing obstruction of the aortic arch or proximal portion of the descending aorta.

Interruption of the aortic arch, distal aortopulmonary septal defect, and aortic origin of the right pulmonary artery with intact interventricular septum were associated findings in a 4-month-old infant who underwent successful one-stage repair without the use of any synthetic material, thus allowing for a potential for growth of both the pulmonary artery and the aorta.
In 2 neonates with interrupted arch we anastomosed the mobilized descending aorta directly to the aortic defect of the aortopulmonary window, closing the pulmonary artery with a pericardial patch.
Aortic arch interruption associated with an aortopulmonary window is a rare congenital malformation that needs an early diagnosis and surgical treatment to avoid irreversible pulmonary lesions.

 Interrupted Aortic Arch
Interrupted aortic arch is characterized by interruption of the aortic arch distal to the left subclavian artery (Type A), proximal to the left subclavian artery (Type B), or between the innominate artery and the left common carotid artery (Type C).
Type A interrupted aortic arch without VSD or with a small VSD is usually repaired much like coarctation of the aorta with end-to-end anastomosis (see Coarctation of the Aorta).
As the site of interruption moves more proximally, the patient is more likely to have a nonrestrictive VSD and a one stage repair of the aortic arch and intracardiac anomalies is performed.

Aortic arch interruption   (Site not responding. Last check: 2007-11-01)
Aortic arch interruption (AAI) is a rare disease with estimated incidence of 0.003 per 1000 live-births, it is characterized by complete lack of anatomical continuity between the transverse aortic arch and the descending thoracic aorta.
AAI should be distinguished anatomically from atresia of the aortic arch where continuity between these segments is achieved by an imperforate fibrous strand of various lengths.
Although echocardiography can offer an excellent visualization of the majority of aortic arch anomalies, angiocardiography remains the gold standard in the diagnosis of this disease.

OBJECTIVE: We investigated the advantages of reconstructing the aortic arch in neonates and infants by end-to-side anastomosis (ESA) in aortic coarctation and type A aortic arch interruption.
SUBJECTS AND METHODS: The patients were 28 neonates and infants who underwent median full sternotomy for correction of aortic coarctation (CoA) and type A interrupted aortic arch (IAA), at our institute, from October 1997 to September 2002.
CONCLUSION: ESA is a relatively simple method for treating CoA or IAA with hypoplastic aortic arch, and was found to be effective for the elimination of residual stenosis.
