AATS: 2005 Postgraduate Course Abstract: The Role of Aortic Root Disease: Implications for Treatment of Aortic Stenosis ...
The aortic annulus, the aortic cusps, the aortic sinuses, and the sinotubular junction are the components of the aortic root and they function as a unit.
The aortic annulus is a fibrous structure that attaches the aortic root to the left ventricle.
Histological examination of the aortic annulus reveals that the aortic root has a fibrous continuity with the anterior leaflet of the mitral valve and membranous septum, and it is attached to muscular septum through fibrous strands.

Aortic sinus - Wikipedia, the free encyclopedia
aortic sinus is one of the anatomic dilations of the ascending aorta which occurs at the aortic root, i.e., just above the aortic valve.
The left aortic sinus gives rise to the left coronary artery.
Usually, no vessels arise from the posterior aortic sinus, which is therefore known as the non-coronary sinus.

The Ross Procedure -- Experts' Techniques -- CTSNet
Between the commissure between the right and non-coronary sinus and the adjacent commissure between the non-coronary and left coronary sinus, the reduction sutures are in the membranous septum, close to the aortic annulus, to avoid injury to the conduction system.
The suture at the left coronary sinus is tied and a continuous suture line attaches the aortic annulus to the autograft posteriorly and this suture is tied to the suture in the non-coronary sinus of the aorta.
The graft is anastomosed to the pulmonary autograft 4 to 5 mm distal to the sinotubular junction of the autograft.

eMedicine - Sinus of Valsalva Aneurysm : Article Excerpt by: Edward J Bayne, MD
Sinus of Valsalva aneurysm is associated with a ventricular septal defect in approximately 40% of patients.
Aneurysms most often involve the right aortic sinus (67-85% of patients, often associated with a supracristal ventricular septal defect), followed by the noncoronary sinus, while an aneurysm of the left sinus is rare.
Under the strain of aortic pressure, the sinus gradually weakens and dilates, causing the formation of an aneurysm.

Journal of Cardiothoracic Surgery | Full text | Post-stenotic aortic dilatation
Aortic root dimensions at the annulus, sinus of Valsalva, sinotubular junction (STJ) and the proximal ascending aorta were assessed in the parasternal long-axis view in systole.
Aortic dilatation associated with haemodynamically normal AV Nkomo et al [25] carried out a community-based study to determine whether the association between BAV and aortic dilatation could be demonstrated in patients with BAV, without significant stenosis or regurgitation.
The pulmonary and aortic roots share a common embryological origin and it has been proposed that the dilatation of the pulmonary autograft may occur as a result of an intrinsic abnormality within the wall, as seen in the aorta of patients with congenital AV disease [57].

Coaptation loss of the aortic valve leaflets was thought to be the cause of regurgitation and congestive heart failure, a rare complication of the unruptured sinus of Valsalva aneurysm.
We diagnosed the patient as having aortic regurgitation with a mycotic aneurysm of the non-coronary sinus of Valsalva due to infectious endocarditis.
Despite severe aortic incompetence prior to surgery it was possible to resect the aneurysm and repair the aortic root without resort to aortic valve replacement and thus return the anatomy to normal.

Patients with anomalous left main coronary artery arising from the right sinus of Valsalva and coursing posteriorly between the aorta and the pulmonary artery are prone to sudden death.
Anomalous origin of the left main coronary artery originating from the right or the anterior sinus of Valsalva has been associated with myocardial ischemia, myocardial infarction, and sudden death.[1-4] Up until now, the conditions of these patients were diagnosed only by coronary angiography, which still remains the gold standard.
Electrocardiogram revealed sinus rhythm and T-wave inversions in leads [V.sub.1] to [V.sub.3.] Coronary angiography revealed that both the right and left coronary arteries arose from the right sinus of Valsalva with two separate ostia that were very close to each other.
