absence of cells of the myenteric plexus BQ'
Examination of tissue obtained on biopsy of the rectum shows absence of cells of the myenteric plexus.
Hirschsprung Disease (colonic aganglionosis) results from a congenital absence of ganglion cells in the myenteric plexus of the colon.
The aganglionic segment extends from the rectum for a variable distance proximally; in 70% of cases the aganglionic segment involves the rectosigmoid colon.

The myenteric ganglia are somewhat irregularly distributed in a complex plexus of nerve fibres [Kapoor 1995].In most portions of the gut, the myenteric ganglia are located around the entire circumference between the outer longitudinal and inner circular layers.
This is due to a developmental disorder characterized by excessive number of nerve fibres that are abnormally distributed affecting the myenteric and submucosal plexus along with hyperganglionosis [Fadda et al 1987, Puri 1997].

 Radiology Case
A small amount of contrast was introduced by a rectal tube, since removed, and notice the beak-like termination of the sigmoid just above the aganglionic segment.
The diagnosis was confirmed by a rectal suction biopsy of the mucosa in this region, which showed an absence of cells of Meisners and Auerbachs plexus.
The patient was treated first with a transverse colostomy to decompress the colon and then the definitive recto-anal pull through operation in which the aganglionic segment is resected while preserving the nerve supply to the anal sphincter.

This image shows a myenteric (Auerbach's) plexus sandwiched between the outer longitudinal and inner circular smooth muscle layers of the muscularis externa.
Note in the magnified view that each plexus contains a ganglion (collection of nerve cell bodies).
