Topic: Autograft

The Ross Procedure -- Experts' Techniques -- CTSNet
When the autograft is used as a root replacement, all adventitia is left on the autograft and the proximal musculature attached to the pulmonary valve annulus is trimmed in a plane 3 to 4 mm below the nadir of the three coronary sinuses.
The autograft is reverted and trimmed for the distal anastomosis, leaving 3 to 4 mm of pulmonary artery distal to the sinotubular junction.
The graft is anastomosed to the pulmonary autograft 4 to 5 mm distal to the sinotubular junction of the autograft.

In the Media - Acceptance and versatility of the Ross Procedure
The ongoing study of the pulmonary autograft supports the use of the Ross procedure in young patients, in females of childbearing age, and in patients with congenital aortic stenosis and complex left ventricular outflow tract obstruction.
The autograft was found to have no increase in regurgitation from pre-exercise to postexercise and gradients were similar to the normal aortic valve.
Their conclusions were that the autograft performed very similar to the normal aortic valve at all levels of exercise.

Bone graft options for a spine fusion
Autograft bone graft is the term used for the patient’s own bone is used to for the bone graft in the spine fusion.
The iliac crest autograft is considered to be the gold standard and is most often used as bone graft in a spine fusion.
At present, this is the most commonly used alternative to autograft bone, and is most commonly used as a bone graft supplement (to the patient’s own bone) in the back of the spine in adult patients.

Should I have allograft or autograft bone?: FAQs - NeckReference.com   (Site not responding. Last check: 2007-10-09)
The decision whether or not to use allograft or autograft bone is based upon many factors including both the patient's smoking history, the patient's age, the degree of osteoporosis and the use or non-use of an anterior cervical plate.
The primary advantage to autograft is that the fusion rates are slightly higher, however, there is a significant amount of pain associated with obtaining the bone from the patient's hip.
While autograft clearly works better to produce fusion, allograft also may be adequate in some circumstances, and allow a patient to avoid the second incision and other risks associated with the bone graft harvest site.

 Ross Operation
The autograft is trimmed of all excessive muscle up to within 1 or 2 mm of the cusp insertions, and any loose tissue is removed from the pulmonary arterial wall.
During insertion, the sinuses of the autograft are carefully matched to the aortic-root sinuses, as this will later ensure a tension-free coronary anastomosis.
Care is taken not to injure the valve leaflets or the annulus of the autograft during preparation.

The Ross Procedure, Page 2 -- Experts' Techniques -- CTSNet
Autograft valve failure or dysfunction requiring replacement of the autograft valve at the time of operative insertion or reoperation within six months of the original Ross Operation is rare.
Echocardiographic assessment of autograft valve function demonstrates that 97% of the root replacement patients have had no change in their autograft valve function during their period of follow-up and only 1% (3 patients) have had an increase of autograft insufficiency to 3+.
The actuarial freedom from replacement of the autograft valve in the 319 operative survivors is 94% + 2% at 8 years, and in the 168 children in this series the actuarial freedom from replacement of the autograft valve is 98% + 1% at 8 years.

 Anterior cervical spinal fusion surgery
Autograft bone (patient’s own bone) is harvested from the iliac crest (hip).
The principal disadvantage with using autograft bone is that another incision needs to be made over the hip to harvest the bone graft.
Whether an autograft or allograft is used is based mostly on a combination of the surgeon’s and patient’s preference.

The graft may be taken from elsewhere in the patient's extremity (autograft), harvested from a cadaver (allograft) or may be synthetic.
Commonly used autografts are the mid-third of the patellar tendon with bone attached at both ends, one or two medial hamstrings, or the quadriceps tendon with bone at one end.
The course after allograft is the same as autograft except that patients often experience less pain in the first week or two allowing an earlier return to light duty work or school.

This is differentiated because it is anatomically identical to an allograft, but closer to autograft in terms of immunology.
The first reasonable account is of the Indian surgeon Sushruta in the second century B.C., who used autografted skin transplantation in nose reconstruction rhinoplasty.
Centuries later, the Italian surgeon Gaspare Tagliacozzi performed successful skin autografts; he also failed consistently with allografts, offering the first suggestion of rejection centuries before that mechanism could possibly be understood.

In one study, VITOSS eliminated the need for harvested autograft in seven (14%) of 50 patients, indicating that VITOSS could be a viable option for patients with no residual graft in the iliac area (hip bone), or with a decreased quality of graft due to osteoporosis.
The most common form of repair used in spinal fusion or for damaged cancellous bone is the autograft, where the patient's own bone material is harvested from elsewhere in the body (usually the hip bone) and then re-implanted into the affected area.
Autograft requires the patient to undergo a second, potentially painful, surgical procedure to retrieve the additional bone material.

Reconstruction of the Pelvis Using a Segmental Autograft of the Ipsilateral Femur
In cases where Zone II has been resected, the curve of the autograft allows the new acetabulum to be placed in an anatomically correct position, since the curve of the proximal femur matches very well the curvature of the pelvic inlet in the hip bone.
The autograft has to be considered as a weight-bearing element in the gait cycle.
Because of this function, and because of the biomechanics of the pelvis, the autograft is wedged into the area between the sacro-iliac joint and the symphyseal region.

Thin autograft donor sites heal faster and can be harvested more frequently for use in the patient with a massive burn injury.
Artificial Skin is indicated for the post-excisional treatment of life-threatening full-thickness or deep partial-thickness thermal injury where sufficient autograft is not available at the time of excision or not desirable due to the physiological condition of the patient.

AATS: TRACHEAL REPLACEMENT WITH AN AORTIC AUTOGRAFT IN A HUMAN PATIENT
Methods: For the animal studies, fifty-one sheep were operated in 3 successive trials: replacement of the anterior segment of two tracheal rings (n=10) and tracheal replacement using an aortic autograft (n=21), or allograft (n=20).
For the human case we replaced 7cm of a tumoral trachea by an aortic infra-renal autograft harvested during the same operation.
Conclusions: The advantage of the aortic autograft subsitute for the trachea is the resistance to infection and the lack of need for immunosuppression therapy.
