Autogynephilia is the behavioral model describing non-homosexual males who, instead of being physically attracted to other women, are physically attracted to the idea of themselves as a woman (Blanchard 1989).
Given that autogynephilia would appear to be not a fetish but a sexual orientation consisting of self-directed heterosexual arousal, (Blanchard 1992), one would then think that such an orientation would not only have states of arousal but also states of desire and states of sexual frustration, as do typical heterosexual or homosexual orientations.
Given that autogynephilia, when viewed as a misdirected sexual orientation and not simply in terms of explicit autoerotic episodes, can account for gender dysphoria as a whole, with no plausible alternative explanation there is every reason to think that autogynephilia is not merely a symptom of gender dysphoria or concurrent syndrome but the underlying cause.

I wanted to test Blanchard's theory of autogynephilia using a sample that was not recruited through a clinical setting (so people who participated didn't have to worry that what they said would effect their chances of getting hormones/surgery etc.), and also use a group of biological females to compare my results to.
Although autogynephilia was most commonly reported by transsexuals who were sexually attracted to females, or bisexual, it was seldom reported by asexual transsexuals, and reported by a significant number of transsexuals who were exclusively sexually attracted to males.

There's a saying: "When your only tool is a hammer, every problem tends to look like a nail." I mention this because many of my problems with her statements about autogynephilia have to do with some of the medical/scientific justification she uses in her claims.
Second, if what she says is true, then maybe one reason why the idea of an autogynephilia classification really sucks is that organizations like the Clarke are marginalizing the autogynephiles -- these TSs clearly don't think that they're getting what they want out of the organization, and they're complaining a lot.
Transsexuals who are not sexually oriented toward men cannot get the same type of relief in that situation, and might learn to cope with their dysphoria in different ways ¾ say, by thinking about other cross-gendered behaviors such as cross-dressing.

According to Bailey, autogynephilia distinguishes the motivation of these later-in-life transgendered men from the larger group of male-born transgendered persons who have always identified as women.
I have no idea if Bailey is right about autogynephilia applying to the motivation of a significant number of transgendered persons, but I have little doubt about his formulation being rooted in psychological phenomena that real people report.
Bailey isn't claiming that 'a small subset' of transwomen are motivated by autogynephilia; he's claiming that all transwomen who aren't exclusively heterosexual (and he describes this population as 'homosexual', thus demonstrating his disrespect for our identities) are really autogynephilic, and if we claim otherwise we must be lying.
