The thing about Azeroth that strikes me is that it has a history, one in which there are game-irrelevant historical details, there are historical characters who don't influence the gameworld, and in which there are events and occurrences in its history that do influence the gameworld.

Then in Warcraft II they added a few other Kingdoms, but still the southern part of the Eastern Kingdoms was always Azeroth Kingdom with the capital of Stormwind.
Besides those two occurences of Azeroth the book always uses Stormwind, though (but in most of the cases it's not clear if the text is meant to refer only to the city or the kingdom of Stormwind).
The section on the changed name of the kingdom can be incorporated into Kingdom of Stormwind; much of it is already there, and also repeated in a third place in Retcon.
