The report into Bueche's death says that the accident occurred while the maintenance team was overhauling the helicopter's main landing gear, including the tires which needed to be replaced.
The investigational report concluded, "the high pressure side of the nitrogen cart (instead of the low pressure side) was attached to the tire, the cart's high pressure controls were found in full flow position and high pressure nitrogen was flowing from the cart to the violently splaying hose that the first responders had to secure."
It is designed to prevent exactly the kind of tragedy that occurred at Balad last year, but like the safety cage and "manpacks," the unit didn't have a working one.
