Antibodies to two basement membrane zone-containing substrates, human epidermis split at the basement membrane zone and monkey esophagus epithelium, are detected by indirect immunofluorescence (IFA).
IgG and complement are found at the basement membrane zone in perilesional tissue, and IgG basement membrane zone antibodies are present in sera in up to 80 percent of patients with localization on the epidermal side of split skin.
Basement membrane zone antibody testing by IFA is a well established test and can be used to distinguish pemphigoid from linear IgA disease (linear IgA bullous disease and childhood immunobullous disease) and epidermolysis bullosa acquisita, as well as other immunobullous diseases such as pemphigus and dermatitis herpetiformis.

Staining with antibody to a basement membrane heparan sulfate proteoglycan core protein related to perlecan did not diminish but rather stained affected tubules intensely, whereas laminin, on the other hand, was apparently diminished in the basement membranes of the cystic tubules.
