  WPXI.com - Health Encyclopedia - Biliary Stricture
Biliary stricture is an abnormal narrowing of the biliary duct, the tube that moves bile (a chemical used in digestion) from the liver to the small intestine.
Biliary strictures are often caused by surgical injury to the bile ducts, for example, after surgery to remove the gallbladder (cholecystectomy).
Depending on the location, the stricture may be surgically removed and the biliary duct rejoined with the small intestine or the hepatic duct.

 Extrahepatic Biliary Atresia
Specimens of excised tissue from the porta hepatis in 26 infants with extrahepatic biliary atresia undergoing hepatic portoenterostomy were analysed histologically for the presence and size of biliary ductules.
Concerning the pathogenesis of biliary atresia, we presume that congenital abnormalities of bile ducts are a basic factor, and additional nonspecific inflammation and bile stasis complete its pathological condition.
Sustained biliary drainage is related to age at operation, the size of biliary ductules at the porta and the subsequent development of cholangitis.

Dr. Koop - Biliary stricture   (Site not responding. Last check: 2007-11-03)
Biliary stricture involves an abnormal narrowing of the biliary duct that transports bile (a chemical used in digestion) from the liver to the small intestine.
Biliary strictures are often caused by surgical trauma to the bile ducts, for example, after surgery to remove the gallbladder (cholecystectomy).
Other causes of this condition are blunt injury to the intestine, pancreatitis, breakdown of the duct by a gallstone in the bile duct, primary sclerosing cholangitis, and cancer of the bile duct.

eMedicine - Biliary Obstruction : Article Excerpt by: Jennifer Lynn Bonheur, MD
Background: Disorders of the biliary tract affect a significant portion of the worldwide population, and the overwhelming majority of cases are attributable to cholelithiasis (gallstones).
In response to a meal, this bile is released from the gallbladder via the cystic duct, which joins the hepatic ducts from the liver to form the CBD.
The clinical setting of cholestasis or failure of biliary flow may be due to biliary obstruction by mechanical means or by metabolic factors in the hepatic cells.

 Biliary Stenting Information on Healthline
A biliary stent is a plastic or metal tube that is inserted into a bile duct to relieve narrowing of the duct (also called bile duct stricture).
Biliary stenting is used to treat obstructions that occur in the bile ducts.
A biliary stent is a thin, tube-like structure that is used to support a narrowed part of the bile duct and prevent the reformation of the stricture.

MUSC DDC - Pancreas & Biliary Tree
The pancreatic juices are made in the acinar tissues, and passed by secretion into the tubes (ducts) for transport into the second part of the upper small intestine (duodenum).
The biliary juices (bile) are made in the tissues of the liver (hepatic parenchyma), and then pass into the biliary ductal system (picture).
The bile duct (biliary) system provides the channels through which bile is transported from the liver to the duodenum (through the papilla of Vater).

Biliary Atresia is a defect in the development of the bile ducts that drain from the liver into the intestines that is characterized by obliteration of the biliary system, resulting in obstruction to bile flow.
Ducts within the liver, extending to the portal hepatis, are initially patent in the most prevalent type III histopathological variant during the first few weeks of life but may progressively be destroyed by the same agent(s) that damaged the extrahepatic ducts and by the effects of retained toxins in bile.

 Hepaticocystic Duct
The classical textbook anatomy of the biliary tract is seen in less than 50 % of cases, and the extrahepatic biliary system is said to have more anomalies in one cubic centimeter of the space around the region of the cystic duct than any other part of the body.
Anomalies of the extrahepatic biliary tree may arise from the gallbladder, cystic duct, hepatic ducts or the common bile duct as a result of aberrations of the normal embryological development, and it is necessary to be familiar with them in order to avoid the disastrous consequences that may follow misrecognition at the time of surgery.
Drainage of both the ducts into a roux loop of the jejunum (as roux-en-y hepaticojejunostomy), partial cholecystectomy and choledochoplasty using a cuff of the gallbladder, or, direct suturing of the common hepatic duct to the cystic duct are the available options.
