Biliary pain resulting from motility disorders is common and may be overlooked due to the difficulty of diagnosing the presence of these disorders.
Because the tests used to evaluate biliary function are technically difficult and not well standardized, the characterization of biliary motor disorders and their associated clinical syndromes remains controversial.
A recent report demonstrated that significant bacterial contamination of bile and the presence of inflammation and fibrosis in the proximal extrahepatic biliary tree were common in patients studied 15 to 17 years after sphincterotomy [24].

The Biliary Tract Cancer International Workshop, held in July 2004 in Shanghai and sponsored by the National Cancer Institute (NCI), Office of Rare Diseases of the National Institutes of Health, Shanghai Cancer Institute (SCI), and the Shanghai Medical Association, was a great success.
The workshop was the culmination of a long-term collaboration between NCI and SCI and showcased novel results of a 4-year collaborative population-based study of biliary tract cancer, the largest and most comprehensive study ever.
DCEG scientists presented two talks and nine posters at the Workshop, reporting positive associations of biliary tract cancer with gallstones, obesity, parity (among women), cholecystitis, chronic infection with hepatitis B virus, a family history of gallstones, a history of liver cirrhosis, and consumption of preserved foods, as well as an inverse association with NSAID use.

Morphologically, cholestasis is characterised by the accumulation of bile in liver cells and biliary passages; biochemically, by the retention of all substances normally excreted in the bile.
Presentation is usually with pruritus and the slow onset of jaundice.

eMedicine - Biliary Atresia : Article by Steven M Schwarz, MD, FAAP, FACN   (Site not responding. Last check: 2007-10-29)
Patients with biliary atresia may be subdivided into 2 distinct forms, which are (1) those with isolated biliary atresia (postnatal form), accounting for 65-90% of cases, and (2) patients with associated situs inversus or polysplenia/asplenia with or without other congenital anomalies (fetal/embryonic form), comprising 10-35% of cases.
In cases in which biliary patency is associated with ductal hypoplasia, further surgical intervention is not indicated, and bile may be collected to evaluate for disorders of bile acid metabolism.
Although their role in the postoperative management of biliary atresia has never been established, some centers have found that short-term, high-dose therapy in the immediate postoperative period may improve the likelihood of achieving adequate bile drainage, particularly for infants in whom bile flow is not immediately apparent following portoenterostomy.

 Gallbladder and Biliary Tract Disease
Some patients experience biliary colic, an intermittent and often severe pain in the epigastrium or right upper quadrant and at times between the scapula caused by temporary obstruction of the cystic duct with a gallstone.
One of the most common causes of extrahepatic biliary obstruction is choledocholithiasis with one or more stones in the common bile duct or common hepatic duct causing biliary obstruction.
Once biliary dilatation or the presence of a common duct stone is noted on an imaging study, or biliary obstruction is strongly suspected on clinical grounds despite negative imaging studies, endoscopic retrograde cholangiopancreatography (ERC) is recommended.
