The bispectral index (BIS) is one of several systems used in anesthesiology as of 2003 to measure the effects of specific anesthetic drugs on the brain and to track changes in the patient's level of sedation or hypnosis.
In technical terms, the bispectral index itself is a complex mathematical algorithm that allows a computer inside an anesthesia monitor to analyze data from a patient's electroencephalogram (EEG) during surgery.
Bispectral analysis is a method of analyzing the mathematical relationships among the various components of an EEG signal (phase couplings) as well as measuring amplitudes and frequencies.

Bispectral index (BIS) monitors are modern neurophysiologic monitoring devices that continually analyse patients' electroencephalograms during general anaesthesia to assess the level of anaesthesia.
BIS monitors may also reduce the incidence of intraoperative awareness in high risk procedures or patients(see discussion [1]) and may have an additional role predicting recovery from severe brain injury though this is far from clear at present.
The bispectral index of an electroencephalogram is a weighted sum of electroencephalographic subparameters including a time domain, a frequency domain, and higher order spectral information (Bispectral Analysis).

Although BIS monitoring may reduce the incidence of intraoperative awareness generally for patients undergoing anesthesia, it has limited value for individual patients for the following reasons: - It is a relatively good indicator of the state of being alert, but its algorithm is inaccurate for predicting an unconscious state.
In both of the large studies that demonstrated a lower incidence of intraoperative awareness (i.e., the Bi-Aware RCT and European prospective study), the anesthesia was deep during maintenance (a mean BIS score of 44.5 [SD, 6.8]in the Bi-Aware trial, and a mean BIS score of 38 [SD, 8]in the European study).
Considering the low sensitivity of BIS monitoring to detect the state of being asleep, several patients would be at risk of receiving unnecessary hypnotic medications to save one from intraoperative awareness.

University Department of Anaesthesia, Glasgow Royal Infirmary, Glasgow, UK We have built a computerised system for closed-loop control of anaesthesia, and present the results of a study of its performance in 10 patients undergoing total hip replacements under general anaesthesia supplemented by epidural anaesthesia.
In the first few cases we experienced problems with oscillation and found that control improved if a shorter bispectral smoothing interval was used.
Later we also altered the control algorithm to take account of the burst suppression ratio, because in some cases there was a paradoxical increase in BIS during burst suppression.
