When you're using a power drill or bolting by hand on rappel there's no excuse for placing a flimsy bolt, and at a minimum you should use a 3/8inch by 2 1/2inch bolt in hard rock, 3/8inch by 3 1/2inches in medium rock, and a 1/2inch by 4 1/2inch glue-in bolt in soft rock.
Climbing bolts should be strong, durable, easy to place, corrosion resistant, have some leeway for user error, and be removable so you can replace them.
We also tested these bolts in soft rock, but their expansion cones, being slightly larger than the drilled hole, sheared away the inside of the hole as the bolt was tapped in, causing the bolt to spin when we attempted to tighten it down.

If, decades in the future, a bolt fails and a second nicely spaced bolt saves the fall no will throw blame, but if the bolt fails and the next bolt is too far down to provide real protection, then people will hunt down anyone they could find to blame.

Bolts were terrible so were also replaced with stainless wedge anchors.
There are still bolts that need to be replace on this crag.

The glue used for glue in bolts, if proper type and correctly used, will last basically forever - tests on glue in bolts in French limestone showed no deterioration in strength after 20 years.
If you replace the hanger only, make sure it is the same material that is used in the bolt, so it won't create a chemical reaction with two different materials.
Climbing, slacklining, mountaineering, and other activities discussed on this site are dangerous.

The nut on the bolt was finger tight and I could easily spin it off and on.
When the bolt is first installed (and tightened) is when the torque wrench would come in handy.
I have found a few bolts, even on walls like Lurking Fear on El Cap and NW Face Half Dome, where the bolt had never been tightened, and was wiggling loose in the hole.

I have not had much of a chance to climb in the spring and the summer was very hot, again with closures.
The climb is solid 5.12b, with the crux moves made on tiny sharp (or small and rounded...) edges with very poor feet, finishing with a dyno from small crimpers to a bucket on the right of the third bolt.
Finally consider that you can always choose not to use a bolt when it is present, but its removal denies any choice to sports climbers and effectively prevents them from enjoying the route.
