  Acromesomelic Dysplasia
Acromesomelic Dysplasia is an extremely rare inherited progressive disorder characterized by premature fusion of the regions (metaphyses) where the shafts (diaphyses) of certain long bones (i.e., bones of the arms and legs) meet their growing ends (epiphyses).
Acromesomelic Dysplasia is inherited as an autosomal recessive genetic trait.
In individuals with Acromesomelic Dysplasia, because the abnormalities of cartilage and bone development in the hands and feet are also progressive, the bones within the fingers and toes (phalanges), as well as in the body of the hands (metacarpals) and feet (metatarsals), may become increasingly shorter and more broad during the first years of life.

Deformities of the wrist are usually operated on around 6 months of age.
Treatment is difficult - mild cases - strapping and manipulation; severe cases - correction of wrist deformity (Centralisation) by fusing ulna to carpus or circular frame and provision of a thumb (Pollicisation) using Buck-Gramko method (transfer, shortening and rotation of index finger).
This is not associated with systemic disorders as is the radial club hand but can be associated with other musculoskeletal deformities, especially hand deformities.

JCA shows many of teh features of adult RA, but some have other features that are almost pathognomonic ie periosteal rxn along the shafts of proximal phalanges and metacarpals, joint ankylosis in wrist and interphalangeal articulations.
There is joint effusion, osteoporosis, symmetrical joint space narrowing, articular erosions, widened intercondylar notch, squared patella, it is very similar to the changes seen in JRA.
Achondroplasia produces skull anomalies including brachycephaly and a hypoplastic skull base, vertebral anomalies including posterior scalloping, scoliosis, laminar thickening, rounded anterior beaking of the lumbar vertebral bodies, and widened intervertebral foramina, yet lumbar spinal stenosis from ventrodorsal and interpediculate space narrowing.
