The basic idea of a breakdown voltage tester is to apply current limited high voltage to an insulator, and raise the voltage until the desired voltage test level is reached, or until a small amount (1 to 2 micro Amps) of leakage current flows.
The light bulb in the primary of the high voltage transformer is an old but good method of limiting the current to the transformer primary in the event of a major component failure.
Since high voltages are involved, it's necessary to build this tester with plenty of open space between components and wiring.
