Breech Presentation - Childbirth Resources at StorkNet's Childbirth Cubby
During pregnancy, a fetus is said to be in a breech presentation when the buttocks of the baby are presenting first at the bottom of the uterus, and the head is in the upper part, or fundus of the uterus.
A breech presentation might be found during a routine prenatal visit when the nurse-midwife or physician feels the mother's abdomen for the outline of the fetus.
There are a number of variations of the breech presentation, determined by the position of the baby's body parts, like arms and legs, in relation to his or her head and trunk.

 BREECH PRESENTATION
Breech presentation is more common when a baby is smaller than average, the uterus is an abnormal shape, there is more than one baby, or the woman has already has children and her uterus is relaxed.
With a complete breech presentation one or both of the legs are extended If the baby is in the incomplete breech position one or both hips are not flexed and the feet and knees are often below the buttocks.

 Baby in the Breech Presentation
Mothers sometimes suspect a breech presentation when the baby's movements are felt very low in the pelvis, or even in the rectum or bladder.
In the case of the breech, it is significant whether one or both legs are folded at the knees ("complete" breech), fully extended with the feet near the face ("frank" breech), or a foot or knee (or two!) descending first.
If he or she is breech near the due date, turning around is still a possibility, although some medical studies find as much as 80% of breech babies stay breech until birth.

eMedicine - Breech Presentation : Article by Richard Fischer, MD
Vaginal breech deliveries were previously the norm until 1959 when it was proposed that all breech presentations should be delivered abdominally to reduce perinatal morbidity and mortality.
Total breech extraction for the singleton breech is associated with a birth injury rate of 25% and a mortality rate of approximately 10%.
Women with breech presentation, reassuring fetal heart rate tracings, and no contraindications to vaginal delivery at 36 weeks' gestation and beyond are usually candidates for ECV (see Contraindications).

IS BREECH PRESENTATION IN NULLIPAROUS WOMEN AT TERM
Background: The aim of this report was to study the perinatal outcome of singleton breech presentation in nulliparous women who delivered after 37 weeks, either vaginally or by cesarean section, and whether by elective surgery or emergency, and to identify the indications for cesarean sections.
Breech delivery was allowed in all nulliparous women unless there was a contraindication for vaginal delivery, such as a large baby (when the clinical and ultrasound estimation of the fetal weight was >3500 g), antepartum hemorrhage, fetal distress, footling breech, failure to progress in labor, previous uterine scar, severe preeclampsia and long-term infertility.
Breech presentation in general and in nulliparous women in particular at term has been managed in different ways, such as external cephalic version, elective cesarean section, and a trial of vaginal delivery.

Breech presentation means that the baby is in a position where the head is the last to emerge, and the buttocks or feet are the first to emerge during delivery.
In a research study published by JAMA (The Journal of the American Medical Association), 130 mothers with breech presentation were treated with moxabustion and an equal number in a control group received no treatment.

Pregnancy Today - Definitions: Breech Presentation
In a breech presentation, the baby is positioned head up, and, if delivered vaginally, would be born buttocks first or with one or both feet first.
Breech positioning is relatively uncommon, occurring in fewer than five percent of all births.
Breech isn't the only unusual presentation: a baby that's posterior is head down but facing forward, which could lead to a stalled labor and "back labor." In a face presentation, the baby is looking down the birth canal with its neck extended.

Breech Presentation - Caesarean operation versus normal birth, by Gina Lowdon
Although Confino, et al found that umbilical cord prolapse was much more common in breech presentations (3.7%) than in vertex presentations (0.3%)(5), cord prolapse may not necessarily be as devastating in breech presentations as it is in vertex presentations, because the fetal legs may shield the prolapsed cord from compression.
High rates of caesarean section for breech presenting babies have not been proved to improve outcome statistics, but the risks involved in operative delivery remain largely unacknowledged, summarily dismissed and are presented on the whole as being generally more acceptable.
Since breech presentations are relatively rare such a person is likely to be one of the more experienced midwives or doctors at the hospital.

Although breech fetuses often reposition spontaneously to a cephalic position up to the 30th to 32nd week of pregnancy, spontaneous cephalic version rarely occurs after this period of pregnancy.
External version, which is a method of rotating a fetus positioned in a breech presentation using external force, may cause premature placental abruption or neonatal asphyxia (Kinoshita et al., 1974; Levinson et al., 1987), resulting in its infrequent use in recent years.
