This particular question - whether mila is the actual cutting or the preparations which facilitate the cutting, sparked the famous controversy whether ideally a father should perform his son's mila rather than delegating it to another.
Mila is definitely zman grama but conceivably could be a zman grama for which women would be chayav were it not for a special pasuk.
But mila which is performed on another and has no pertinence to her, I might suggest she would be chayav, just like Beit Din which must circumcise children whose father was derelict - for this reason the pasuk teaches me that women are patur.) This ambiguous and multifaceted Ramban allows several meanings.

You wrote it yourself, Mikhvah is the final requirement for a Ger Tzedek or Proselyte and not circumcision.
Zest: FIRST comes the circumcision (and yes, if the converting gentile did have it done earlier it must now be done symbolically with just a drop of blood drawn, known as hatafat dam brit), and then the FINAL STEP is the immersion in the waters of the mikveh, as far as I know.
Why would you think that a man can "skip" brit mila (the required circumcison of conversion), and fancy to call himself a Halakhic "Jew" by merely going into a mikveh?

I guess I was naïve enough to think that most Jews were familiar with the brit and the differences between the procedures.  To say it simply, most Jews I have talked to had NO IDEA of the real facts.
First, Brit Mila is, in fact, the most humane procedure if one is to do a circumcision at all.  Second, it is my opinion that even medically the potential benefits outweigh the potential negatives.
So we see that traditional brit mila is, in fact, a very 'advanced' procedure, already encompassing beneficial baby-comforting devices doctors are only now learning.  Not bad for a ritual that is thousands of years old.

There are those who hold that metzitza b'peh is an integral part of brit mila - I guess that means that according to this view one has not fulfilled the mitzva unless this component of the procedure has been performed.
When a circumcision was not initially performed leshem mitzva, one transforms it into a brit mila through hatafat dam brit as Frank described (there is actually a machlochet about it, but this is certainly the accepted halacha).
That may be the logical extension of the hard-line position on metzitza b'peh, which is probably not a position that any of these folks would acknoledge that they are advocating.
