Bronchiolar Airway Disorders and Bronchiolitis Obliterans
Bronchiolitis obliterans organizing pneumonia (BOOP) was described in 19851 as a distinct entity, with different clinical, radiographic, and prognostic features than the airway disorder obliterative bronchiolitis2 and the interstitial fibrotic lung disorder usual interstitial pneumonia/idiopathic pulmonary fibrosis (UIP/IPF).

 bronchiolitis obliterans
By utilizing this definition bronchiolitis obliterans is usually an irreversible process (in contrast to the reversibility of granulation tissue) and therefore has a strong correlation with diminished pulmonary function test scores, primarily forced expiratory volume at one second.
Bronchiolitis obliterans develops on an average of 11 months post-transplant, however it has been noted to develop as early as two months after transplant and as late as several years after transplantation.
eveloping hand-in-hand with bronchiolitis obliterans is a progressive myointimal thickening of the pulmonary arteries and veins which corresponds to the graft atherosclerosis seen in the coronary arteries of heart allografts.

s020409a - Bronchiolitis Obliterans Organizing Pneumonia (BOOP)
Organizing pneumonia was classified into 3 clinical groups: symptomatic cryptogenic OP; symptomatic OP related to underlying hematologic malignant neoplasm, collagen vascular disease, or drugs (secondary OP); and asymptomatic OP presenting as a focal nodule (focal OP).
Organizing pneumonia was an asymptomatic focal rounded opacity in 10 patients (14%), most often detected on chest radiograph and diagnosed on lung biopsy done for suspicion of lung cancer.
Abstract: Idiopathic bronchiolitis obliterans with organizing pneumonia (BOOP) is a clinicopathologic syndrome characterized by an indolent course and favorable prognosis.

BOOP has long been known to be associated with connective tissue diseases and in the now classic paper by Epler et al, a connective tissue disease was present in about 10% of patients with BOOP.
BOOP tends to be patchy and in typical cases, areas of involvement are separated by relatively normal lung.

Bronchiolitis obliterans organizing pneumonia (BOOP) was the term proposed in 1985 by Epler and colleagues for a distinctive clinicopathological syndrome that had been previously described under a variety of terms including cryptogenic organizing pneumonitis, bronchiolitis obliterans with classical interstitial pneumonia (BIP), and bronchiolitis obliterans and organizing pneumonia-like process.
An acute and chronic bronchiolitis is usually present away from the areas of intraluminal fibrosis and consists of a peribronchiolar infiltrate of acute and chronc inflammatory cells associated with varying degrees of epithelial necrosis.
Interstitial pneumonia is present in most cases and is characterized by an alveolar septal infiltrate of mononuclear inflammatory cells with varying degrees of septal thickening and epithelial hyperplasia.

Bronchiolitis obliterans organizing pneumonia (BOOP) is a clinicopathologic syndrome characterized by excessive proliferation of granulation tissue forming plugs in the small airways and alveolar lumina in association with a surrounding chronic inflammatory cell infiltrate.
BOOP can be idiopathic and has been described in association with bacterial, viral, parasitic, and fungal infections, connective tissue diseases, drug toxicity, or toxic gas inhalation although cryptogenic types continue to be the most common forms.

Background: Organizing pneumonia is characterized by the presence of granulation tissue in the distal air spaces.
Organizing pneumonia may be classified according to whether (1) its cause is determined, (2) its cause is undetermined but occurring in a specific and relevant context, or (3) it is cryptogenic (idiopathic) organizing pneumonia (COP) (Cordier, 2000).
These were associated with patchy areas of organizing pneumonia consisting largely of mononuclear cells and foamy macrophages in the surrounding lung.

Bronchiolitis obliterans organizing pneumonia (BOOP) is a pathologic finding common to various injuries to the lung of either definite or idiopathic etiology.
This has been designated as "cryptogenic organizing pneumonitis,"[3] "organizing pneumonia-like process,"[4] and "bronchiolitis obliterans organizing pneumonia" (BOOP).[5] The last terminology is now largely accepted, although "intraluminal fibrosis of distal airspaces" would probably be preferable, since it does not predetermine the bronchiolar or alveolar predominance of the condition.
In the present study we distinguished three distinct, homogeneous clinical and imaging profiles in a series of patients studied at our institution, in which intraluminal fibrosis of distal airspaces was found in specimens of pulmonary tissue in the context of idiopathic pulmonary disease.
