The Confederation of North, Central America and Caribbean Association Football (CONCACAF) was founded in 1961 and serves as the organizing body for 40 national associations, including the United States, and spans from Canada in the north to Suriname in the south.
CONCACAF organizes international competition in a number of events, including the Champions' Cup, qualifying tournaments for all FIFA competitions and the Gold Cup.
CONCACAF competed in the FIFA World Club Championship for the first time in 2000, when Mexico's Necaxa impressed with a third place finish in the 2000 tournament in Brazil.

In 2002, the 3 CONCACAF teams played very well, and if it were not for goal difference, then Costa Rica would have made it into the knockouts along with the US and Mexico.
Before that in 1994 CONCACAF also had MEX and USA reach round of 16 (but there were only 24 teams in the finals, so it was easier to advance; USA fell to Brazil 1-0, and MEX lost to Bulgaria in penalties).
Mexico has basically single handedly carried CONCACAF to 12 World Cup Appearance and the development of Mexico was stunted in part because CONCACAF was horrific for a long time save for the occassional good Honduras and Guatemala team.

Luis Bueno has an article up at CNNSI where he talks about the difficulties that CONCACAF nations face on the world soccer stage and also talks about they many common ’solutions’ that have been proposed to lift CONCACAF out of the world’s soccer basement.
If CONCACAF ever got strong enough to compete with CONMEBOL on an equal footing, a merger might make sense to counter the larger federations, but if done now, CONCACAF teams would be basement dwellers forever.
CONCACAF has lots of little teams - so would you basically put the decent CONCACAF teams into the draw pool with CONMEBOL and then let the little nations duke it out in their own groups for one slot or a playoff slot?
