Tumoral calcinosis cutis is a familial syndrome of calcium phosphate deposits in close to the joints, significant hyperphosphatemia and normal serum calcium.
Metastatic calcinosis cutis is characterized by widespread calcifications in the viscera and elevated serum calcium and phosphorous, and is associated with chronic renal insufficiency.
Massive dystrophic calcinosis cutis has been described in diabetic patients on insulin, but the lesions are localized to areas of insulin injection, unlike our patient.

Background: Calcinosis cutis is a term used to describe a group of disorders in which calcium deposits form in the skin.
Calcinosis cutis is classified into 4 major types according to etiology: dystrophic, metastatic, iatrogenic, and idiopathic.
The pathogenesis of calcinosis cutis is not completely understood and a variety of factors allow for different clinical scenarios to occur.

SiriusDog.com - Calcinosis Cutis, Calcium Circumscripta, Canine Gout
Calcinosis cutis circumscripta in humans is most often seen as nodules in the skin of the extremities, especially the hands (scleroderma).
If the calcinosis develops in injured tissue, it could be localized, in which case some have surmised it to be often associated with demodex, TB, staph infection, or granuloma caused by a foreign body such as grit or sand imbedded beneath the skin, or it could be connected with epidermoid cysts or malignant tumors.
The calcinosis cutis could be widespread, appearing in any or all of the following: skin along the back, armpit, groin, flanks, over bony protuberances such as foot bones and vertebrae, and (reportedly) apocrine (sweat) glands.

The diagnosis is calcinosis cutis, a condition caused by the deposition of calcium phosphate crystals within the skin.
Calcinosis cutis may be associated with an inflammatory disease, such as acne, and has also been linked to previous dermabrasion.

Dystrophic calcinosis cutis: Calcification is usually localized to a specific area of tissue damage, though it may be generalized in some disorders.
Calcinosis cutis circumscripta generally occurs earlier and tends to involve the extremities, whereas calcinosis universalis occurs later and is usually more widespread.
CT is infrequently used in evaluating calcinosis cutis and primarily used in assessing tumoral calcinosis.

Iatrogenic calcinosis cutis presents with similar symptoms and signs; its diagnosis may be easily confused with an infective condition by an unwary orthopaedic surgeon.
The clinicoradiological diagnosis of this case is iatrogenic calcinosis cutis following intravenous infusion of calcium solution for the management of convulsions.1 Although iatrogenic calcinosis cutis has references in the paediatric and dermatology literature, the majority of orthopaedic surgeons are unaware of this disease.
The dystrophic variety occurs in damaged or traumatised tissues and the serum calcium/phosphorus ratio is found to be normal.4 The metastatic variety occurs in normal tissues in the presence of a disturbed calcium and/or phosphorus balance.4 The rarest variety is iatrogenic calcinosis cutis.

Dystrophic Calcinosis cutis in Subacute Lupus
Dystrophic calcinosis cutis is known to be associated with various connective tissue disorders but to the best of our knowledge has never been reported in subacute cutaneous lupus erythematosus (SCLE), a distinctive cutaneous subset in the spectrum of lupus erythematosus.
We report a patient with SCLE who developed calcinosis cutis and had normal serum calcium and phosphorus levels and, interestingly, a normal concentration of blood ionized calcium.
This latter, which represents the active form in the total amount of blood calcium, is a parameter only rarely assessed in patients with dystrophic calcinosis cutis.
