The real rationale is that, in a monarchy, passports are issued in the name of the monarch to her subjects, asking foreign governments to grant the passport holders free passage, assistance, and protection.

Canadian citizens will not be given a visa for their non-Canadian passport.
I mean, he could travel there on his.ca passport, he could even try to travel there on his US one and just not get it stamped, and there's a good chance you wouldn't have any hassle in the process.

It says" Effective January 04, 2005, we are no longer permitted to add visa pages to passports which have been filled with visa stamps.
There is generally a special unit for citizen's services, one of these is providing new passports and adding pages (I extrapolate from services provided by US embassies).
My wife had a similiar but not the exact same scenario and actually we had a bit of problem getting a new passport since she had more than 6 months remaining on the current passport but they eventually did issue a new passport(what were we to do if they refused to!).
