Dr. Koop - Carcinoid Syndrome- Health Encyclopedia and Reference
Carcinoid syndrome is caused by an uncommon intestinal or lung tumor, called a carcinoid tumor, that secretes excess quantities of hormones.
Symptoms associated with carcinoid tumors (also called argentaffinomas) include attacks of severe flushing of the skin lasting from minutes to days, watery diarrhea, abdominal cramps, bronchoconstrictive attacks, and edema (swelling) of the head and neck.
Metastatic carcinoid tumors may be diagnosed by the clinical findings and by elevated levels of 24-hour urinary 5-hydroxyindolacetic acid (5-HIAA), a metabolic product of the serotonin produced by the tumor.

National Cancer Institute - Gastrointestinal Carcinoid Tumor Treatment
A gastrointestinal carcinoid tumor is cancer that forms in the lining of the gastrointestinal tract.
Carcinoid tumors that start in the rectum (the last several inches of the large intestine) usually do not produce hormones.
The hormones produced by gastrointestinal carcinoid tumors are usually destroyed by blood and liver enzymes.

Carcinoid tumors, except those originating in the rectum, produce a variety of endocrine substances, the most frequent of which are serotonin (5-hydroxytryptamine) and kallikrein (an activator of bradykinin release).
[8] Primary carcinoids of the extrapelvic colon are uncommon, typically present with metastatic disease, and have a poor prognosis.
Carcinoid tumors with gross regional lymphatic metastasis or local extension should be treated by aggressive surgical resection.

Carcinoid Malignancy - Overview, Carcinoid Syndrome & Treatment - oncologychannel
Carcinoid tumors, or carcinoids, originate in hormone-producing cells of the gastrointestinal (GI) tract (i.e., esophagus, stomach, small intestine, colon), the respiratory tract (i.e., lungs, trachea, bronchi), the hepatobiliary system (i.e., pancreas, gallbladder, liver), and the reproductive glands (i.e., testes, ovaries).
Carcinoid tumors of the appendix usually are benign and often occur between the ages of 20 and 40.
Carcinoid tumors that are confined to the GI tract may not cause symptoms (i.e., be asymptomatic) because the hormones produced by these tumors are released into the bloodstream and destroyed by the liver.

In a few cases of bronchial carcinoid tumors, the cancer is located in the very outer part of the lungs and may not cause any symptoms at all.
Carcinoid tumors that develop in the appendix or rectum are usually very slow growing and rarely affect long-term survival.
For carcinoids that develop in other parts of the intestine or stomach, the 5-year survival is 95% if it has not spread, 65% if it has only spread to the local lymph nodes, and about 20% if it has metastasized to the liver.

Carcinoid Tumors: Merck Manual Home Edition
Carcinoid tumors are noncancerous (benign) or cancerous (malignant) growths that produce excessive amounts of hormonelike substances.
When carcinoid tumors occur in the digestive tract or pancreas, the substances they produce are released into a blood vessel that flows directly to the liver (portal vein), where enzymes destroy them.
Carcinoid tumors of the lungs, testes, and ovaries also cause symptoms because the substances they produce bypass the liver and circulate widely in the bloodstream.

Carcinoid tumors, gastrointestinal: Treatment - Health Professional Information [NCI PDQ] -- General Information
The majority of carcinoid tumors are slow-growing tumors that can be treated and often cured, especially in early stages.[1] The occurrence of metastasis from carcinoid tumors relates directly to the size of the primary tumor (lesions 1 cm or less rarely metastasize; lesions greater than 2 cm frequently metastasize).
The diagnosis of carcinoid syndrome (a syndrome associated with flushing, diarrhea, bronchoconstriction, cardiac valvular lesions, and telangiectasia) is aided by demonstrating elevated 24-hour urinary 5-hydroxyindoleacetic acid levels.
In patients with the carcinoid syndrome, palliation is sometimes obtained with pharmacologic agents that suppress production or block the action of vasoactive amines; of particular interest is a somatostatin analogue.[13] Some patients benefit from the use of interferon alfa.

The most common symptom of localized carcinoid tumors (those which have not spread beyond the organ it developed in) is abdominal pain caused by "kinking" or blockage of the intestines.
Some patients with carcinoid tumors also have cancers or benign tumors of other organs, so doctors will ask about symptoms that might suggest other tumors are present.
PET scanning for carcinoid tumors uses a radioactive form of 5-hydroxytryptophan, a chemical that is taken up and used by carcinoid cells.

Once the onset of symptoms occurs in the carcinoid syndrome, whether it be flushing or diarrhea, in the untreated patient, the average survival is 3 years.
Patients with the carcinoid syndrome obviously have a worse outlook than those without the syndrome because they have both the growth of the tumor and the problems related to the effects of the circulating hormones on their different organs.

The tumor usually arises in the gastrointestinal tract, anywhere between the stomach and the rectum (the favorite spot is in the appendix) and from there may metastasize (spread) to the liver.
One or more of four kinds of treatment are used for carcinoid tumors: surgery (to take out the cancer); radiation therapy (using high-dose x-rays to kill the cancer cells); biological therapy (using the body's natural immune system to fight the cancer); and chemotherapy (using drugs to kill cancer cells).

ACS :: Can Gastrointestinal Carcinoid Tumors Be Found Early?
Because carcinoid tumors usually grow and spread slowly, about half of all gastrointestinal carcinoid tumors are found at an early or localized stage.
Occasionally, when a person's appendix is removed to treat acute appendicitis (infection and inflammation of the appendix that causes abdominal pain), doctors may incidentally find a small carcinoid, in addition to the expected finding of inflammation.
In most of these cases, the carcinoid was still too small to have caused any problems.
