The pancreas is a retroperitoneal organ located posterior to the stomach on the posterior abdominal wall.
The portal vein is formed by the union of the superior mesenteric vein and splenic vein posterior to the body of the pancreas.
Control of the exocrine function of the pancreas are via the enzymes gastrin, cholecystokinin and secretin, which are enzymes secreted by cells in the stomach and duodenum, in response to distension and/or food and which cause secretion of pancreatic juices.

Acinar cell carcinoma of the pancreas is a rare neoplasm, comprising 1-2 % of exocrine pancreatic tumors (Solcia E et al., 1997).
Ultrastructural studies of acinar cell carcinomas have demonstrated a resemblance to normal pancreatic acinar cells with well-developed endoplasmic reticulum and zymogen granules (mean diameter 400-500nm), which are commonly oriented toward the luminal space.
Acinar cell carcinomas are aggressive tumors and most patients die from their cancer within a mean of 18 months after diagnosis and a 5 year survival of 5.9%; however, the overall survival is better than pancreatic ductal adenocarcinoma (Klimstra DS et al.).

Currently, the WHO recognizes clear cell carcinoma as a 'miscellaneous' carcinoma of the pancreas, which characteristically is rich in glycogen and poor in mucin although adenocarcinomatous components, ductal phenotype, and presence of mutations in the k-ras oncogene are recognized (1).
A handful of cases have been reported as 'clear cell carcinomas of the pancreas' in the setting of anaplastic carcinoma, pleomorphic giant cell carcinoma, solid pseudopapillary tumor, and in a single case of primary intraductal carcinoma with invasive clear cell component (4,5,6,7).
Although clear cell features have been described in the context of anaplastic carcinoma, pleomorphic giant cell carcinoma, solid pseudopapillary tumor, and in a single case of primary intraductal carcinoma with invasive clear cell component, there is a lack of reported cases in the literature for a conventional ductal adenocarcinoma composed of predominantly clear cells (1-8,14).
