Topic: Cardiotoxic

In the News (Wed 30 May 12)

  Resident Report Volume 3, Number 11;  September 10, 2004
To assess whether this estimate is reflective of the incidence in the broader clinical oncology setting, the authors evaluated data from three prospective studies to determine both the incidence of doxorubicin-related CHF and the accumulated dose of doxorubicin at which CHF occurs.
We examined echocardiograms from 120 children and adults who had received cumulative doses of 244 to 550 mg of doxorubicin per square meter of body-surface area for the treatment of acute lymphoblastic leukemia or osteogenic sarcoma in childhood, a mean of 8.1 years earlier.
Cardiotoxicity is a recognized complication of doxorubicin therapy, but the long-term effects of doxorubicin are not well documented.

Cardiotoxic effects of arsenic trioxide/imatinib mesilate combination in rats
Cardiotoxicity is an important consideration in the evaluation of cancer chemotherapy, because chemotherapy-induced myocardial damage might be irreversible and lethal.
Cardiotoxicity was evaluated by biochemical and histopathological examination 48 h after the last dose.
In conclusion, both arsenic trioxide and imatinib mesilate might have significant cardiotoxicity and cardiac function should be monitored during treatment with them alone or in combination, as well as in the presence of pre-existing cardiac dysfunction.

 Chloroquine (PIM 123)
The cardiotoxicity of chloroquine is related to it quinidine-like (membrane/stabilizing) effects.
If cardiotoxic symptoms are present or if the dose ingested is higher than 3 g in adult or 30 mg/kg in child, a continuous infusion of diazepam may be given at a dose of 5 to 10 mg/hour in adults or 0.25 to 0.4 mg/kg/hour in children over a period of 24/48 hours.
Given that chloroquine cardiotoxicity is rarely observed later than 48 hours after ingestion, diazepam administration after the 48th hour appears not to be justified.

 Cardiotoxic Potential of Anti-Emetics (5-HT3)
Nonetheless, the development of significant electrocardiograph changes with some agents may indicate a potential for significant cardiac effects in patients, particularly those who may be predisposed to cardiac complication.
Because two of the most common major comorbidities are cardiovascular related, we need to be acutely aware of the toxic effects of chemotherapy, as well as the possible cardiac interaction of supportive agents, specifically the 5-HT3 antiemetics.
Until more data are made available, the best antiemetic option for patients receiving emetogenic and cardiotoxic chemotherapy may be the agent with the fewest apparent cardiac effects.

Batrachotoxin - Wikipedia, the free encyclopedia
Batrachotoxins are extremely potent cardiotoxic and neurotoxic steroidal alkaloids found in certain species of frogs (poison dart frog), Melyridae beetles and birds (Pitohui, Ifrita kowaldi).
These effects are similar to the cardiotoxic effects of digitalis (digoxin), a poison found in the foxglove plant.
While it is not an antidote, the membrane depolarization can be prevented or reversed by either tetrodotoxin (from puffer fish), which is a noncompetitive inhibitor, or saxitoxin ("red tide").

Reduced cardiotoxicity of DOXIL (pegylated liposomal doxorubicin) in AIDS Kaposi's sarcoma patients compared to a matched control group of cancer patients given doxorubicin (Meeting abstract).
Doxorubicin (DOX)-induced cardiotoxicity can limit the duration of therapy with this agent and may be exacerbated by combination with newer agents such as taxanes.
DOXIL, a pegylated liposome formulation of doxorubicin, is less cardiotoxic than DOX in rats, dogs and rabbits.

That may not always be the case in the intubated patient or the patient with a cardiotoxic overdose.) The expertise of the ED physician must overcome this thought process (or lack thereof).
Sodium channel blockade is the most important factor in toxicity, resulting in slowing of the maximum upstroke slope (Vmax) of phase zero depolarization of the action potential, decreasing conduction velocity in the Purkinje fibers and increasing QRS duration, and decreasing automaticity.
In summary, cardiotoxicity results from multiple effects on the cardiac cell action potential, direct effects on vascular tone and indirect effects mediated by the ANS.

HFSA: Heart Failure Resulting From Trastuzumab (Herceptin) Use Largely Reversible
TORONTO, ON -- September 15, 2004 -- Cardiotoxic events that occur as a result of treatment with the human monoclonal antibody trastuzumab (Herceptin) are largely reversible, according to a study from the MD Anderson Cancer Center, in Houston, Texas.
To determine the true risk of cardiotoxicity associated with trastuzumab, researchers reviewed the charts of 35 patients who were receiving trastuzumab and were referred for evaluation of possible left ventricular (LV) dysfunction.
All had received prior anthracycline-based chemotherapy and were from a much larger population of patients with breast cancer that were treated with trastuzumab at MD Anderson Cancer Center.

Excessive blood levels can be caused by a rapid intravascular injection of a therapeutic dose of drug, but serious toxicity is most often the result of a frank local anesthetic overdose.
The typical patient who succumbs in the dental setting is not an adult administered one of the supposedly more cardiotoxic anesthetics or those implicated in methemoglobinemia.
It is important to realize that narcotic sedative regimens lower the convulsive threshold of local anesthetics and increase the likelihood of respiratory depression and precipitous falls in cardiac output.

Clinical Trial: Epidemiology of Cardiotoxicity in Children With Cancer   (Site not responding. Last check: 2007-10-08)
To provide a comprehensive analysis of risk factors for the development of clinical cardiotoxicities in over 6,000 children with cancer who had been treated on standardized protocols involving the use of anthracyclines alone or in combination with other potentially cardiotoxic therapies or with no use of anthracycline therapy.
The data were analyzed to estimate the incidence of clinical cardiotoxicity as measured by sudden death, congestive heart failure, or discontinuation of therapy based on cardiac function.
The incidence of clinical cardiotoxicity was calculated using Kaplan- Meier estimates as a function of total cumulative anthracycline dose and also as a function of the time since the end of treatment stratified by dose levels.

Of the 1 million Americans hospitalized each year for a heart attack, about 20 percent will also suffer from depression that puts them at triple the risk of dying.
Yet most of these patients are not treated with antidepressants because older antidepressants were known to be cardiotoxic and the effects of the new antidepressants were unknown.
In the first study of an SSRI (selective serotonin reuptake inhibitor) antidepressant in 369 depressed patients recently hospitalized for heart attack, the drug sertraline (Zoloft) has been found to be safe and effective.

Barrier Word Algorithm and Approximate Matching
A potential nominal phrase is computed as a sequence of words occurring between barrier words, which are derived from a set of stopwords including articles, prepositions, and verbs.
The set of barrier words might be used to identify local anesthetic bupivacaine, cardiotoxic, and circulation as nominal phrases.
While this method has been used for some time, the use of a very long list of barrier words (approximately 24,000) was found to be much more effective in identifying nominal phrases in text than the traditional shorter lists.
