Coreg Clinical Pharmacology - Carvedilol - HealthScout
Carvedilol is subject to the effects of genetic polymorphism with poor metabolizers of debrisoquin (a marker for cytochrome P450 2D6) exhibiting 2-to 3-fold higher plasma concentrations of R(+)-carvedilol compared to extensive metabolizers.
Following concomitant administration of carvedilol (25 mg once daily) and digoxin (0.25 mg once daily) for 14 days, steady-state AUC and trough concentrations of digoxin were increased by 14% and 16%, respectively, in 12 hypertensive patients.
Although carvedilol is metabolized primarily by the liver, plasma concentrations of carvedilol have been reported to be increased in patients with renal impairment.

There were also almost 100 fewer hospitalizations among the carvedilol patients as compared with the placebo group.
Carvedilol was found to improve not only the ejection fractions but to decrease the risk of abnormal rhythms, greatly increasing its benefit in these patients.
After 15 months of treatment, the carvedilol group had larger increases in their ejection fractions than the metoprolol group, as well as improvements in various pressures in the circulation to the heart at lungs.

Carvedilol shares a common pathway for elimination by the liver with several other drugs such as quinidine (Quinaglute), fluoxetine (Prozac), paroxetine (Paxil), or propafenone (Rythmol).
Carvedilol may increase the levels in the blood of cyclosporin (Sandimmune, Neoral), and the dose cyclosporin may need to be adjusted when the two drugs are used together.
Carvedilol should be used cautiously in patients who use diuretics or who are elderly or have cirrhosis, asthma, peripheral vascular disease, hyperthyroidism, Prinzmetal's variant angina (angina at rest), and kidney disease.

Carvedilol is effective, safe and well tolerated in patients with severe chronic heart failure, according to the results of a recent multinational trial.
A previous study of carvedilol, the Carvedilol US Trial Programme, showed a 65 per cent reduction in mortality with carvedilol in patients with mild to moderate heart failure.
In a press release, Roche Pharmaceuticals said that carvedilol was not licensed for treatment of severe heart failure in the UK and that a licence application would be made shortly.

HgbA1c in the carvedilol group increased insignificantly from 7.21 to 7.23; in the metoprolol group it increased significantly by 0.15 from 7.19 to 7.
Since carvedilol has both alpha and beta adrenergic blocking activity, whereas metoprolol is only a beta-blocker, it is not a complete surprise that a high dose of metoprolol was required to equal the antihypertensive effect of carvedilol.
It is interesting to note that in the COMET trial, comparing the beneficial effects of carvedilol to metoprolol in patients with CHF, the target dose of carvedilol was 25 mg bid, that of metoprolol was 50 mg bid.

The agent investigated was carvedilol, a non-selective beta-blocker with alpha-1 blocking (vasodilating) and anti-oxidant properties.
Patients randomized to carvedilol were titrated up towards 50 mg bid, except for a subgroup randomized to a dose-ranging study, where the maximum doses were as shown in the figure.
Had these deaths been attributed to carvedilol, they would have accounted for 25% of carvedilol deaths and would have greatly diminished the calculated benefit of the drug.

Furthermore, treatment with carvedilol decreased the incidence of apoptosis in cardiac myocytes from failing hearts at both doses.
These data suggest that the inhibition of apoptosis with carvedilol may lead to improvement in ventricular function and may underlie a beneficial effect of β-blockade independent of heart rate lowering effects.
