Topic: Causes of hypoglycemia

Hypoglycemia - Wikipedia, the free encyclopedia
Hypoglycemia (hypoglycæmia in the UK) is a medical term referring to a pathologic state produced by a lower than normal amount of sugar (glucose) in the blood.
When hypoglycemia occurs repeatedly, a record or "diary" of the spells over several months, noting the circumstances of each spell (time of day, relation to last meal, nature of last meal, response to carbohydrate, and so forth) may be useful in recognizing the nature and cause of the hypoglycemia.
Hypoglycemia is also a term of contemporary American folk medicine which refers to a recurrent state of symptoms of altered mood and subjective cognitive efficiency, sometimes accompanied by adrenergic symptoms, but not necessarily by measured low blood glucose.

Hypoglycemia should always be excluded as the cause of an initial episode of convulsions, coma, or a sudden alteration of neurobehavioral functioning.
Hypoglycemia occurs within two hours of glucose ingestion and is associated with an even earlier hyperglycemic phase due to the rapid absorption of glucose with prompt insulin secretion.
The term functional hypoglycemia has been used to describe such patients, many of whom may have emotional complaints that are unrelated to the prevailing blood glucose concentration and whose symptom complex may be triggered by placebo rather than by glucose.

Hypoglycemia can be caused by abnormal function of the hormones that regulate blood sugar or by the inability of the body to store adequate amounts of glucose.
The cause of Hypoandrenocorticism is unknown, although immune-mediated destruction of the adrenal gland is suspected in most cases.
A diagnosis of hypoglycemia as a cause of neurological problems is based on the presentation of clinical signs of hypoglycemia, blood glucose concentration test shows levels below normal, and the fact that clinical signs go away when glucose is administered to the patient.

Hypoglycemia may be a condition by itself, or may be a complication of diabetes or another disorder.
When a child with diabetes has symptoms of hypoglycemia, then the cause is usually diagnosed as a complication of diabetes, or insulin reaction.

Hypoglycemia is most commonly a complication of diabetes treatment (diabetic hypoglycemia).
In rare cases, hypoglycemia may be caused by inherited enzyme or hormone deficiencies, especially those that affect the metabolism of sugars and other carbohydrates.
Hypoglycemia also may occur, though rarely, under certain conditions in early pregnancy or with prolonged fasting or missed meals, severe malnutrition, or prolonged strenuous exercise, such as running a marathon.

By far the commonest disorder of this kind in children is ketotic hypoglycemia, which occurs in young children when the carbohydrate intake is reduced, often as a result of diminished appetite in a minor illness, to the point that fat stores are needed for energy and ketones are produced.
Reactive hypoglycemia is different from spontaneous hypoglycemia, which is not associated with meal ingestion.
Identification of the possible cause of hypoglycemia and selection of diagnostic modalities are facilitated by considering whether the patient appears to be healthy (with or without compensated coexistent disease) or ill (with a disease known to have a proclivity to develop hypoglycemia, or is hospitalized).

An excessively high blood insulin level (hyperinsulinemia) may be caused by a rare disorder of the beta cells in your pancreas, where insulin is released.

Some tumors don't cause an overproduction of insulin, but cause excessive utilization of glucose by the tumor or they result in an overproduction of insulin-like substances.

The most common cause of mild or severe hypoglycemia in childhood is insulin-treated type 1 diabetes and a mismatch among food, exercise, and insulin.
Hypoglycemia occurs when one or more of these counterregulatory mechanisms fail, either through glucose overutilization as in hyperinsulinism, underproduction as in the glycogen storage diseases, or a combination of the two as in growth hormone or cortisol deficiency.
During infancy, hyperinsulinemia, hypopituitarism, and inborn errors of metabolism are frequent causes of hypoglycemia.
