Cavus Foot | Orthopedics | Children's Hospital and Regional Medical Center
Cavus foot in one foot is more common in children with injuries to the nerves in their legs or spinal cord or to the muscles in their legs.
In the very early stages of cavus foot or with mild cases that are not progressing, we may be able to use arch supports and shoe modifications to help relieve problems until surgery becomes necessary.
When we perform reconstruction surgery for cavus foot, our main concern is to correct the deformities without joining together any joints (fusion) and to balance out the muscles so that the problem is less likely to happen again.

Anterior pes cavus which is the plantarflexion of the forefoot on the rearfoot or a rearfoot pes cavus which is when the rearfoot is dorsiflexed excessively on the forefoot.
Fixed cavus may be satisfactorily reduced by multiple dorsiflexory osteotomies of the metatarsals The advantage of these procedures are that the rearfoot and lesser tarsal joints are preserved with post operative disability less (Simons, 1994).
This procedure entails a cresentric, biplane osteotomy at the posterior aspect of the calcaneus that permits the reduction of the calcaneal inclination angle or sagittal plane deformity as well as frontal plane deformity by rotating the posterior portion of the calcaneus dorsally (Drvaric, 1994).

The treatment of the cause was to correct to a precise degree, the function of the foot during the gait cycle.
This foot type is prone to inversion sprains because the rigid forefoot valgus forces the hindfoot into inversion when the patient bears his or her weight, and this stresses the lateral aspect of the ankle.
The excessive pounding that takes place with the cavus foot can result in painful periostitis of the tibia, which, by being already bowed, is subject to considerable stresses in running (three to four times the body weight combined with about 1,000 foot strikes per mile).

In all cavus feet the front part of the foot is drawn downwards more than normal, and the part on the side of the big toe (first ray) is usually drawn down most of all.
Cavus feet tend to be stiffer than normal and may not take pressure as well as normal feet, so they amy ache if you have been on your feet for a while.
However, if your cavus feet cause a lot of pain, rub badly on your shoes so that the skin breaks down, or your foot or ankle are very unstable, and simple treatment has not helped, it may be worth considering an operation to straighten your foot.

Pes Cavus | High Arch Shoes
Pes Cavus (commonly referred to as high arches) is a term used to describe a foot with excessive toe to heel arch elevation.
The condition of high arches is often associated with abnormal orthopedic and neurological conditions or occurs with the onset of neuromuscular disease that diminishes muscle tone in the foot.
The best treatment for pes cavus is to follow your doctors prescribed recommendations and wear high arch shoes designed to alleviate the pain of a pes cavus condition.

In an April 1994 rating decision, entitlement to an increased rating for dermatophytosis of the feet, entitlement to service connection for a skin disorder of the buttocks, secondary to service-connected dermatophytosis of the feet, and entitlement to a total disability evaluation for compensation based on individual unemployability, were denied.
The diagnoses were status post external fixation trauma to the left calcaneus post tibia, fibula fracture of the left leg; neurological deficit of sharp/dull and tactile sensation with deep penetration; and range of motion limitation of sub talor joint, mid tarsal joint, and metatarsal phalangeal joints 1-5 of the left foot with crepitation noted.
The appeal is granted as to the issue of entitlement to an increased rating for a left tibial nerve injury, secondary to service-connected residuals of a fracture of the left tibia and fibula with shortening and pes cavus, to the extent indicated, subject to the law and regulations governing the award of monetary benefits.

Practitioners surmount pes cavus treatment challenge   (Site not responding. Last check: 2007-09-19)
Pes cavus is the general term used to describe a high-arched foot, but the condition typically involves various foot abnormalities, including rearfoot varus, forefoot equinus, metatarsus adductus, and digital deformities.
Chronic ankle pain is another common cavus complaint, as the rearfoot varus and forefoot valgus associated with the deformity leave patients vulnerable to lateral ankle trauma and recurrent sprains.
When conducting a clinical examination of the patient with a pes cavus foot, the goal is to define the deformity and determine which components are flexible.

Cavus (kv´us) foot deformity is also known as “pes cavus” or, more simply, high-arches.
In most cases, at the early stage of cavus foot, the tilt in the heel is relatively flexible.  After time though the joints may become stiff and the tilt may no longer be flexible.
Cavus foot may run in families.  It may also be caused by other medical conditions that cause muscle weakness such as Spina Bifida or muscular dystrophy.  Or the condition may be part of a progressive neurological disease.

The cavus foot, also called pes cavus or high arches, results from a muscle imbalance between the extrinsic and intrinsic muscles of the foot.
A variety of muscle transfers in various combinations (such as moving the long peroneal muscle to the short; or moving the posterior tibialis to the dorsum of the foot) may also be done to eliminate the deforming force and improve the position of the foot.
Surgical procedures to fix rigid claw toes include transferring the tendons connected to the toes to the bones of the foot, fusing interphalangeal joints (the ones in the middle of the toes) and fusing bones.

In cavus deformity resulting from poliomyelitis, preliminary data indicate a positive effect in feet with pure cavus deformity and cavus deformity associated with equinus angulation of the hind part of the foot, while the results in feet with a cavus deformity and associated calcaneal deformity of the hind part of the foot typically were unsatisfactory.
