Cefotetan, like cefoxitin, is used for the treatment of urinary tract, lower respiratory tract, skin and skin structure, bone and joint, gynecologic and intra-abdominal infections caused by susceptible organisms.
Cefotetan has a half-life of 2.8 to 4.2 hours3 after IV or IM administration as opposed to cefoxitin with a half-life of approximately 1 hour;l therefore, cefotetan can be dosed every 12 hours instead of every 6 to 8 hours.
Cefotetan is 88% plasma protein bound and is 51 to 81% excreted unchanged by the kidneys over a 24-hour period.2 The principal nonrenal route of elimination is biliary excretion; approximately 20% of a dose is reportedly excreted in the bile.1

To compare the effectiveness of cefotetan administered at 2 g once a day with cefoxitin at 1 or 2 g three times a day in the treatment of hospitalized patients with skin and superficial soft tissue infections, 194 patients from eight centers were enrolled in an open, randomized trial.
The incidence of adverse reactions in the cefotetan group (17 percent) was significantly different from that for the cefoxitin group (6 percent) (p less than 0.05); however, the incidence of treatment-related reactions was not significant and the events were mild.
Bacteriologically, 97% of the 58 evaluable patients in the cefotetan group and 89% of the 27 evaluable patients in the cefoxitin group had a satisfactory or presumed satisfactory response; two patients in the cefotetan group and three in the cefoxitin group were considered bacteriologic failures.

Recent reports in the literature show increasing number of drug induced immune hemolytic anemia associated with cephalosporin (38/43 including 35 cases with prophylactic use of cefotetan) and 29 % resulted in fatal issue when the diagnosis was delayed.
