From these studies, the combination of 1% zinc gluconate, 0.2% zinc acetate and 0.2% zinc lactate is superior to all single zinc salts tested and to other zinc salt combinations tested in preventing the penetration of the dye through the lubricant base and its binding to the surface of the skin.
Because zinc gluconate has been shown to prevent latex allergies, this salt was tested alone and also in combination with zinc lactate and zinc acetate, which represented one of the better combinations of zinc salts identified in example 2 and still contained an amount of zinc gluconate sufficient to protect against allergic reactions to latex.

Several recent trials have compared the efficacy of povidine-iodine with chlorhexidine gluconate in reducing catheter colonization and bloodstream infection with mixed results.
The summary relative risk of bloodstream infection for all vascular catheters was 0.44 (95% CI: 0.25-0.78), indicating a significant benefit in patients using chlorhexidine gluconate.
However, given that the cost of chlorhexidine gluconate is currently 6-fold greater than the cost of povidone-iodine (30 cents vs 5 cents per 5 cc of solution), a formal economic evaluation will be required to determine the economic consequences of using chlorhexidine gluconate routinely for catheter site care.

The risk of vascular catheter-related infection is reduced when chlorhexidine gluconate is used for cleaning insertion site rather than povidone-iodine solution.
Site care with chlorhexidine was associated with a 51 percent reduction in the risk of catheter-related bloodstream infection compared with povidone-iodine.
With this case scenario chlorhexidine gluconate should be considered as a replacement for povidone-iodine solution, particularly in patients at high risk for catheter-related bloodstream infection.

The objective of the study was to evaluate the cost-effectiveness of using chlorhexidine gluconate and povidone iodine for vascular catheter insertion site care, to clarify for decision-makers the clinical and economic consequences of using each of these solutions.
The authors reported that different types of chlorhexidine gluconate solution were used in the individual trials from which the measure of effectiveness was derived.
They also estimated the incremental cost of chlorhexidine gluconate over povidone iodine under a variety of clinical situations, as different packages and amounts of povidone-iodine solution are used for catheter site care, and alcohol solution is sometimes applied before the catheter site is disinfected with povidone iodine.

Using chlorhexidine gluconate solution for vascular catheter site care greatly reduces the risk of catheter-related infection
The good news is that use of chlorhexidine gluconate in place of the current standard solution (povidone iodine) to clean the site of catheter insertion reduces the risk of catheter-related bloodstream infection by half, according to a study supported in part by the Agency for Healthcare Research and Quality (HS11540).
Based on the model, they estimated that use of chlorhexidine rather than povidone reduced central catheter-related bloodstream infections by half (a decline from 31 to 15 cases per 1,000 catheters) and decreased infection-related deaths by 0.23 percent at a savings of $113 per catheter used.
