Chronotherapy Aids Chemotherapy   (Site not responding. Last check: 2007-10-08)
According to the report, chronotherapy reduced the rate of the severe effects of the treatment on the lining of the gastrointestinal tract by five-fold, and cut the incidence of peripheral neuropathy (a side effect of some chemotherapy drugs that causes numbness of fingertips and feet) by 50%.
The group of patients receiving chronotherapy also had significantly fewer bouts with other side effects of the treatment, including inflammation of the mouth (six times less), severe diarrhea (two times less), and moderate hair loss (seen in 5% of the constant-rate group, none of the chronotherapy group).
The results in the chronotherapy group of patients were so significant that the researchers, on advice from their policy board, stopped the trial early.

A Time to Heal: Chronotherapy Tunes In to Body's Rhythms
Chronotherapy for asthma is aimed at getting maximal effect from bronchodilator medications during the early morning hours.
Chronotherapy for all forms of arthritis uses standard treatment, nonsteroidal anti-inflammatory drugs and corticosteroids; however, the dosages are timed to ensure that the highest blood levels of the drug coincide with peak pain.
But chronotherapy has a way to go, considering that only 5 percent of the doctors surveyed said they were "very familiar" with the subject.

"Chronotherapy is the key to [asthma] management," says Dr. Richard Martin, an asthma specialist at National Jewish Center for Immunology and Respiratory Medicine in Denver, in the April 15, 1996 edition of Hospital Practice.
Chronotherapy has also been used to lower the amount of side effects from chemotherapy drugs.
Chronotherapy may be a fairly new concept, but it is certainly not an outlandish one.

Evolving Concepts in Chronotherapy with CCBs: Implications for Pharmacists
Chronotherapy is a method of optimizing safe and effective pharmacotherapy by proportioning drug concentration throughout the 24-hour time period in synchrony with biologic rhythms of disease processes.
A chronotherapeutic approach may be chosen when the threat of dangerous medical events or the intensity and symptoms of a disease are known to vary predictably during the 24-hour time period.
In the cardiovascular patient, the focus of chronotherapy would be to optimally deliver the antihypertensive and/or antianginal agent in higher concentrations during the time of greatest need (ie, the early morning postawakening hours) and lower concentrations when the need is less (ie, the middle of the sleep cycle).

Chronotherapy was first used to treat DSPS in 1981 and was first suggested by C.A. eisler.
In chronotherapy, the patient progressively goes to bed and wakes up 3 hours later than the previous night, until he or she moves around the clock and can consistently sleep earlier.

Chronotherapy is difficult, although for people with DSPS it is often much easier than just trying to wake up at 7 am from the first day.
During chronotherapy, patients often can fall asleep very quickly as soon as they get into bed, which helps their confidence.
Be sure to check with your doctor if you have another condition that chronotherapy could interfere with, such as if you're taking insulin or other regular medication.

Chronotherapy reduced the rate of severe adverse effects on the lining of the gastrointestinal tract (eg, sloughing and inflammation) five-fold and halved the rate of functional impairment from peripheral nerve damage, such as loss of sensitivity of fingertips or feet.
Median time to treatment failure was 6.4 months for those on chronotherapy and 4.9 months for those on constant infusion, which is a significant difference.
The investigators conclude that chronotherapy was significantly less toxic and more effective than flat infusion and that the concept of circadian selectivity of cancer chemotherapy is well founded.

This last form of treatment is known as "acupuncture", whose purpose is to re-establish the normal circulation of Qi within the meridian, and, subsequently, the affected organ.
Chronotherapy in TCM encompasses the recognition of the diurnal variation in the flow of Qi throughout the internal organs and the treatment of imbalance, by acupuncture, at specific times.
One of the classic and most important works written on TCM is "The Yellow Emperor's Classic of Medicine", thought to have been written by the emperor Huang Di, who reigned during the middle of the third millenium BCE (Ni, 1995).

Clearly, our goal in chronotherapy protocols for each of these diseases was to reduce treatment-related toxicity and complication rates by optimal circadian drug timing, allowing high-doses of drug to be administered safely and most effectively.
In a 5-day chronotherapy protocol, the infusion rates of FU, LV and I-OHP were automatically increased and decreased along the 24 hour scale using a multi-channel pump (Intelliject, Ivion Corp., Englewood, CO).
