Chyle fistula is defined as a leakage of lymphatic fluid from the lymphatic vessels, typically accumulating in the thoracic or abdominal cavities, but occasionally manifesting as an external fistula.
Chyle fistulas also can form as a result of venous hypertension, and they have been described in patients with superior vena cava syndrome or thrombosis of the vena cava, among others.
For chyle fistulas to form, either a scarcity of lymphatic collaterals must be present or the injury to the lymphatic channels must overwhelm the remaining lymphatic vessels.

The enriched lymph originating in the lymphatics of the small intestine is called chyle (not chyme).
The nutrients that are released to the circulatory system are processed by the liver, having passed through the systemic circulation.

The cisterna chyli is a reservoir that then funnels chyle into the thoracic duct, which traverses the thoracic cavity along the aorta and empties into the venous circulation near the heart.
In the last decade, new treatments have been limited to several drug treatments introduced to reduce the volume of chyle formation (rutin and octreotide), which have not proven to be uniformly effective in resolving chylothorax in dogs.

The main components of chyle are as follows: protein, predominately albumin - 2 to 4.5 gm/% (1/2 - 2/3 serum protein); Na, K+, Cl-, Glucose - values like plasma; WBC's - 1K-20K cu/mm (mostly T lymphocytes); and fat - 1% to 3% or more.
As discussed earlier, the flow and production of chyle are particularly dependent on the amount of dietary fat ingested and chyle quantity is noted to decrease when dietary fat is eliminated.
