The Cisalpino is a high-speed train that interconnects Stuttgart, Basel, Geneva, Zürich, Milan, Venice, and Florence.
The tilting train (colloquially known as "Pendolino"), the Italian ETR 470, provides a comfortable and fast connection between Italy, Switzerland, and Germany.
The Cisalpino is operated by the Cisalpino AG which has its headquarters in Bern.

Cisalpino postponed revenue service from June 2, 1996, the start of the summer timetable, to the winter schedule to give engineers time to reprogram the tilting software.
The Cisalpino company formed by the Swiss Federal Railways (SBB), the Bern-Lotschberg-Simplon Railroad (BLS) and the Italian State Railroads (FS) financed the $20 million fleet of ETR 470 Pendolino trains.
Cisalpino considers this an efficient deployment of its high-priced equipment because the line between Zurich and Stuttgart is so highly curved that a tilting train will very effectively reduce travel time.

Reaching top speeds of 155 mph and providing capacity for 430 passengers, the seven-car trainsets will be added to Cisalpino’s existing fleet, which has been in service since 1996 between Switzerland, Italy and Germany.
Delivery is expected to begin in May 2007 and be complete in January 2008.

Cisalpino trains require reservations before boarding on international runs, though inside Switzerland you can just get on them and not pay the reservation fee.
You can also purchase a seat reservation on the Cisalpino train within Italy (if you can't meet the time limits for seat reservations from the station of boarding), although this is not the preferred method, and you may need to hunt around for an empty seat.
Normally, I would completely agree that using the Cisalpino service is ideal...HOWEVER, depending on the time of year/date of travel the FIRST Cisalpino which runs all the way to Spiez without changes does not depart Milan until 5:10 PM so a better option might be taking a non-Cisalpino service.

While my Cisalpino consisted of nine carriages, three first class, one dining and five second class, the Zurich-Stuttgart ICE T consisted of five carriages, one first class with lounge, three second class and one with a cafe plus second-class seating.
Whereas Cisalpino devotes a full carriage to the best and most tasteful dining area to be found on the trains of Europe, the 5-carriage ICE T has only a token cafe with no seats.

The Eurostar (I believe this applies in the Italian Eurostar but you might want to research this) is a variation of the French TGV, made by Alstom, and does not tilt.
The Cisalpino, manufactured by Fiat (now owned by Bombardier, who also now owns Alstom and built the Amtrak Acela), has a hydraulic tilt system and can travel up to 20% faster on conventional lines.

Agree with the others that the train from Geneva is much less hassle, unless the price difference to Milan is very great, in which case take a taxi from Milan airport to Milan Centrale station and get the super-fast Cisalpino train to Brig and then the little red train up to Zermatt.
You would of course have to compare prices on the train from Milan versus the train from Geneva to see if the cheaper airfare is worth it.
IF you take a Cisalpino train which is about 15 minutes or so faster than the regular trains on this route, and requires seat reservations, you will have to go all the way to Brig to change since it will not stop in Visp.

After having ridden only the Italian Cisalpino up to now, we knew we had unofficially switched countries the minute we stepped onto the the German-made ICE.

John Clayton here with news about an amazing, wonderful train, and an equally magnificent lunch I was served when, about 7 weeks ago, I rode the luxurious Cisalpino train from Zurich to Lugano, in Switzerland.
All was confirmed by the stunning Swiss sideshow of lakes, mountains and greener than green fields, as they swept by each side of the train.
My lunch salad was so fresh, I was sure it'd been picked as the Cisalpino flashed by a luscious looking vegetable garden, and my scaloppini was among the best I've ever tasted.
